This role supports the implementation of the Future Cares Initiative.  The Future Cares Initiative is part of the LIFEspan Service with a focus on supporting clients and caregivers with longer term planning and assist with successful community transition and integration. The Community Support Workers focus is on supporting the complex needs of clients and their loved ones in creating a longterm plan that includes but is not limited to subject areas such as legal supports housing social / recreational meaningful activities employment / volunteer opportunities physical and mental health care.  The successful candidate will accomplish this by providing onetoone instrumental supports to LIFEspan clients and their loved ones advocating for clients and families around community resources as well as facilitating both psychosocial groups and psychoeducational workshops within these subject areas as deemed appropriate. The CSW participates in program planning and quality improvement activities; engages in program and hospital professional development; and collaborates with interprofessional teams as well as communitybased and private service providers.< / p><p> < / p><p><strong>Key Responsibilities< / strong>< / p><p><strong>LIFEspan Client Support : < / strong>< / p><ul><li>Provide information and guidance on available resources and services tailored to each clients goals and needs.< / li><li>Support clients in developing their own life plans or guides based on whats important to them.< / li><li>Help clients navigate the Coriat Cares Fund application process.< / li>< / ul><p> < / p><p><strong>Project Assistance : < / strong>< / p><ul><li>Collaborate with the Future Cares Initiative team to implement project initiatives.< / li><li>Maintain uptodate client records and documentation related to future care planning and fund applications.< / li><li>Assist in monitoring and evaluating the effectiveness of support services provided.< / li>< / ul><p> < / p><p><strong>Resource Development : < / strong>< / p><ul><li>Develop informational resources for LIFEspan clients and their loved ones.< / li><li>Ensure all resources are accessible userfriendly and meet the diverse needs of our client population.< / li>< / ul><p> < / p><p><strong>Workshop Coordination : < / strong>< / p><ul><li>Assist in the planning organization and implementing of workshops.< / li><li>Coordinate logistics materials preparation and participant communication.< / li><li>Support outreach efforts to encourage LIFEspan client and caregiver participation.< / li>< / ul><p> < / p><p><strong>Administrative Duties : < / strong>< / p><ul><li>Manage scheduling and appointment setting for LIFEspan client meetings held either virtually or by phone or inperson.< / li><li>Assist in general inquiries via phone and email in a professional and timely manner.< / li>< / ul><br / ><p><b>Qualifications : < / b> < / p><p><ul><li>Social Service Worker diploma or an equivalent combination of education and experience< / li><li>Completion of a Bachelors Degree in Social Work Arts Science or equivalent is an asset< / li><li>Registration with the College of Social Workers and Social Service Workers is required< / li><li>Experience and comfort level with conversations related to death / dying (such as previous experience in a palliative care setting) and planning for this inevitability in the context of dependent adult children< / li><li>Experience with facilitating psychosocial and / or psychoeducational groups< / li><li>Experience navigating community resources and essential services< / li><li>Computer proficiency < / li><li>Knowledge of policies and programs related to income insecurity and access to housing< / li><li>Strong verbal / written communication and advocacy skills< / li><li>Knowledge and / or experience of navigating physical and developmental supports and services is an asset< / li><li>Previous experience working with people living with cerebral palsy and / or acquired brain injuries is preferred< / li>< / ul>< / p><br / ><p><b>Additional Information : < / b> < / p><p><p>In addition to working alongside some of the most talented and inspiring healthcare professionals in the world UHN offers a wide range of benefits programs and perks.
