Overview
We are currently hiring a Part-Time Registered Practical Nurse (RPN) and / or Registered Nurse (RN) for Community Home Care in the Central West and Oakville area. This role requires availability for both day shifts and night shifts. A $1,500.00 sign-on bonus is offered.
You will be part of a diverse and collaborative care team that provides client-centered home and community care. You will use evidence-informed nursing knowledge and judgment within the RPN / RN scope of practice to deliver holistic care addressing the physical, safety, emotional, and spiritual needs of clients and their families in their homes and communities. You may have opportunities to provide point-of-care leadership as the primary provider for assigned client populations.
Responsibilities
- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs
- If designated as the primary provider for a client caseload, serve as the point-of-care lead and collaborate with other care team members and support functions
- Support clients through care transitions by coordinating with community support services in their local area
