Job Purpose :
Reporting to the Pharmacy Manager, the Clinical Pharmacist is responsible for supporting professional services for patients visiting the pharmacy care clinic. The Clinical Pharmacist works with the healthcare team to provide prescribing services, point of care testing, comprehensive medication reviews, delivers educational activities and promotes pharmacy initiatives in order to achieve desired health outcomes.

Principal Responsibilities :
Manage and oversee operations of Pharmacy Care Clinic
Complete Patient assessments and if needed prescribe new medications / extend existing medications
Travel Health Consultations
Chronic Disease Management
Conduct Patient Medication Reviews on an Annual basis and complete follow ups where required
Responsible for ensuring care clinics processes and documentation meets regulatory compliance
Administration of medications by injection
Point of Care screening / swabs
Providing lab requisitions and interpreting lab results (if provincial legislation allows)
Collaboration with other HCPs to establish circles of care for patients
Counselling patients on prescription medication use
Supporting dispensary activity and pharmacy workflow activity
Collaboration with other health care providers to maintain and ensure continuity of care.
Assist in developing and implementing industry leading clinical services and programs
Maintain solid understanding of industry trends, laws, and regulations regarding pharmacist expanded scope and care clinics
Perform other duties as assigned to support Rexall Pharmacy Group Ltd. including supporting operational responsibilities as required.
Knowledge, skills and experience :
Bachelor of Science in Pharmacy, fully licensed in the Province of employment and in good standing
Must be Injection Certified
Additional accreditations are an asset (e.g., CDE, CRE, CTH, etc.)
Must have prescribing authorization
Highly organized, takes initiative and problem solving and decision-making skills
Effective planning and time management skills
Minimal direction and work independently
Internal Communications :
Participate in continuous learning activities and share relevant takeaways with other staff
Build a strong cross functional team and working relationships with various colleagues across RSC
External Communications :
Strong interpersonal skills, with a dedication to providing outstanding customer service to clients
Ability to optimally deliver presentations
