Position Summary :
The Coordinator for the Toldo Neurobehavioral Institute (TNI) provides both operational and clinical oversight and expertise. The TNI Coordinator will report to the Operations Manager (OM) and work in collaboration with the Clinical Practice Manager (CPM) in order supervise the program delivery, day to day operations, and patient care. The TNI Coordinator will support the CPM in the identification, facilitation and adoption of best practice and quality improvement initiatives that support Hotel-Dieu Grace Healthcare’s pursuit of service excellence in mental health and addictions.
The TNI Coordinator will supervise the coordination of patient care services and use effective communication and problem-solving skills to ensure optimal patient outcomes. The TNI Coordinator will provide supervision to ensure effective transitions of care, and guidance to staff as required in order to support quality and safe patient care. The TNI Coordinator is responsible for providing leadership and direction to the entire multidisciplinary team. The TNI Coordinator will oversee the accurate and timely completion and collection of the Resident Assessment Instruments (RAIs) in alignment with ministry mandates
Qualifications : Responsibilities :
- Ensures collaboration amongst the multidisciplinary team to coordinate patient care plans and supervises programming schedules and service needs. Ensures that clinically mandated programming is delivered, and that elective options are arranged for patients.
- Attends multidisciplinary rounds in collaboration with OM, communicates information to staff on patient care plans to promote and facilitate continuity of care
- Evaluates staff compliance with respect to following and updating patient care plans
- Oversees Safety Huddle daily on the units – updates safety / risk mitigation strategies in real time
- Oversees the accurate, timely completion of RAIs
- Ensure compliance related to initiation and maintenance requirements for patients using Clozapine in collaboration with GenCan
- Supervises the coordination of patient appointments to ensure a consistent and organized approach around staff accompaniment and transportation with adherence to policy to procedure
- Facilitates the implementation of the quality standards in collaboration with administration
- Performs routine unit audits (i.e ‘Just Clean Your Hands’)
- Liaises with Administration, the Intake Nurse, and Security if required in the development, implementation, evaluation and sustainment of safety plans.
- Collaborates with the Decision Support Analyst to advance program data collection and reporting while ensuring it is both timely and accurate
- Work in collaboration with the Decision Support Team to review quality scorecards, and that reporting mandates are met.
- Analyze scorecard results to order to generate, develop and initiate Quality Improvement initiatives
- Consult the Unit Based Council and Patient and Family Advisory group as necessary
- Participates within relevant hospital committees for quality improvement purposes
