Our community‑based clinic Our Community Advantage Rehabilitation (CAR) offers family‑centered services to adults and children in their homes and schools.
Responsibilities
- Provide ministry‑funded and private client physiotherapy services in the client’s residence or via virtual services.
- Conduct assessments (interviews, standardized and informal techniques) in gross motor skills, coordination, neuro‑developmental, and mobility aids.
- Maintain accurate documentation related to assessment findings, care plans, progress, and discharge summaries according to CAR, the referral source, and College of Physiotherapists of Ontario standards.
- Develop personalized, evidence‑based care plans to support individual rehabilitation and wellness goals.
- Collaborate with a multidisciplinary team to coordinate patient‑centric care and achieve optimal outcomes.
- Provide mentorship and leadership to junior staff, as assigned.
- Engage in continuous learning and professional development to stay current with best practices.
- Use modern tools, including e‑charting and digital communication, to streamline workflow.
- Partner with therapy assistants and kinesiologists to maximize clinical impact.
- Educate patients and families on recovery, prevention, and self‑management strategies, linking to community agencies when appropriate.
