The Registered Practical Nurse (RPN) reports to the Director of Care and is expected to coordinate and oversee all aspects of resident-focused care delivery. RPNs should also promote the values, goals, and philosophy of the Home.

Responsibilities :

Develop and modify care plans based on residents’ individual healthcare needs and changes in health status.

Liaise with healthcare personnel, agencies, and families.

Maintain good relationships with team members, residents, families, and the public.

Engage in meaningful interactions with residents during shifts.

Supervise activities on the unit and take charge as required.

Monitor performance and complete evaluations for PSWs.

Direct, assign, and evaluate PSW work.

Possibly be in charge of the Home as needed, utilizing leadership skills to identify and solve problems.

Ensure care is delivered respecting the Resident Bill of Rights.

Understand and participate in quality indicator improvements.

Listen to and resolve resident and family concerns according to Home policies.

Transcribe and process physician's orders for medication and treatments per CNO standards.

Order and manage nursing supplies and equipment efficiently.

Conduct rounds, administer medications, and document all care accurately and timely.

Complete assessments, develop, and update care plans collaboratively.

Participate in resident care conferences and maintain comprehensive records.

Assess health status changes and communicate with SDM and interdisciplinary team.

Perform other assigned tasks as needed.

Qualifications :

Applicants must have :

Current Certificate of Competence with the College of Nurses of Ontario as an RPN or equivalent registration in another Canadian jurisdiction.

Basic CPR certification (asset).

Experience in long-term care (asset).

Ability to work well with interdisciplinary teams.

Strong communication skills.

Knowledge of RAI-MDS; certification required within one year if not already certified.

Clear vulnerable sector screening (within 6 months), TB test results (within 6 months), and three supervisory references.
