Job descriptionExcellent interpersonal communication networking and negotiation skills. The ability to prioritise delegate and organise tasks effectively. Experience managing client budgets and knowledge of Home Care Packages Aged Care Standards and community service networks for older Australians and their carers. Diploma or degree in Health / Allied Health or 6 years relevant experience. Strong communication advocacy and interpersonal skills. Proven time management prioritisation and budget management ability. Knowledge of upcoming Aged Care sector changes. Current Australian drivers licence and reliable vehicle. We have an exciting opportunity for a part-time (0.8 FTE) Care Manager to join our Care Finder team in Abbotsford .

About the role
This is a part-time maximum-term position until 30 June 2026 for an experienced and enthusiastic community care professional. You will provide independent ongoing advice and guidance to clients across the Eastern Melbourne region .
Key responsibilities include :
- Delivering assertive outreach to identify engage and build rapport with potential clients.
- Building networks with local intermediaries to strengthen client access to services.
- Taking a multi-dimensional approach to assessing client needs.
- Supporting clients to access My Aged Care packages and other relevant community services.
- Promoting Care Connects service model and value proposition.
- Providing customer service excellence to clients and their representatives.
