Position Summary The Clinical Consultant Pharmacist (CCP) is responsible to draw from geriatric‑focused clinical practice guidelines and collaborate inside multidisciplinary teams to deliver clinical recommendations and process‑guidance for a portfolio of residential care, assisted living, and independent living homes to optimize for resident outcomes, safety, and regulatory compliance. The compensation is salary. The environment is hybrid work‑from‑home and on‑site at the care homes.

The CCP works in partnership with the clinical team, operations, home partners, and other departments in the CareRx organization. This position is FULL‑TIME 5 days per week and is located in the Kelowna area, with periodic travel to Lake Country, Vernon, Salmon Arm, and Williams Lake. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Residential Care Regulations, Community Care & Assisted Living Act, site policies, and internal CareRx policies.

Responsibilities

  • Act as a liaison between long‑term care homes and retirement communities and CareRx Pharmacy to ensure optimal resident care, pharmacy services, and clinical support.
  • Travel to LTC homes and retirement communities.
  • Build and maintain relationships within the community to ensure ongoing collaboration and communication between home / community leadership teams and pharmacy teams.
  • Complete medication reviews in currently serviced retirement communities and long‑term care homes serviced by CareRx Pharmacy.
  • Complete all required documentation and billing processes as required by MOHLTC, College of Pharmacists of British Columbia and other federal and provincial standards and regulations.
  • Provide education to residents and staff on various health‑related topics including the Meds Check program and attend community‑based wellness fairs and other events as required.
  • Conduct education sessions within Long Term Care Homes and Retirement Communities within scope of practice, such as policy and procedure, web portal, eMAR, influenza protocol, etc.
  • Support CareRx Pharmacy policies and procedures within retirement communities and long‑term care homes.
  • Complete medication audits on‑site in LTC homes and retirement communities as required.
  • Ensure regular on‑site visits are made to all assigned clients.
  • Attend / participate in annual professional conferences and trade shows, when applicable.
  • Participate in the development and implementation of industry‑leading clinical pharmacy service, when applicable.
  • Be an education, policy and pharmacy initiative resource.
  • Be available for special clinical team projects as required.
  • Collaborate in the development of medication audit follow‑up improvement plans when applicable.
  • Work collaboratively with other team members to support applicable homes and communities.
  • Participate in pharmacy‑related transition processes (e.g., new client transitions, eMAR transitions, etc.).
  • Help with pharmacy operations / drug distribution when required.
  • Attend and actively participate in clinical team meetings.
  • Qualifications

  • Insured and currently licensed in good standing with the College of Pharmacists of British Columbia.
  • Proven ability to foster strong partnerships with other members of the healthcare team.
  • Effective organization, planning, and time‑management skills.
  • Knowledge of Kroll Windows application and Microsoft Office programs.
  • Complete understanding of provincial regulations governing LTC homes and retirement communities.
  • Must be familiar with provincial standards of practice established for LTC pharmacies.
  • Excellent customer service, interpersonal skills, oral and written communication skills and ability to interact with persons of varying levels of professional expertise.
  • Strong presentation and teaching skills.
  • Self‑directed, requiring minimal supervision and able to work independently.
  • Motivated to achieve results that reflect a high standard of quality.
  • Good listening skills and adaptability to changes in job environment.
  • Dependable and responsible in the execution of job duties.
  • Adaptable to changes within internal work structures.
  • Must possess a valid driver’s licence.

    Compensation

    Annual salary : $109,200 - $119,600. Actual pay rate will be based on the individual candidate’s credentials and experience.

    Location and Work Shifts

    Primary location will be LTC homes and retirement communities in the Kelowna Interior Region—Kelowna, Lake Country, Vernon, Salmon Arm, Lumby, and Williams Lake. This is a full‑time position working 5 days a week with flexibility and opportunity to work‑from‑home and on‑site from the care homes.
