Nurse Practitioner

Position :

Nurse Practitioner (NP)- Full Time Permanent.

Executive Director, Owner / Operator

Position Summary :

• The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of SRR’s inter-professional Care Team.
• The NP lead clinical services would include ongoing assessments that will be documented in the plan of care as well as a review and an update of the plan of care for each resident at least twice per year.
• The NP will complete a comprehensive geriatric assessment (CGA) for the management of care for frail seniors.
• The Nurse Practitioner will coordinate and work in collaboration with inter-professionals such as the other members of the care team, a social worker, an occupational therapist, a pharmacist, and a BSO Clinician to offer geriatric clinical integrated care to support frail older adults and seniors who live at SRR.
• The NP will provide direct patient care focusing on promoting health, maximizing patient safety, and function that will support frail seniors living in the retirement home community.
• The Nurse Practitioner functions within their expanded scope in a highly autonomous manner to use their expert skill to formulate clinical decisions and appropriate diagnosis and manage acute / chronic illness and wellness promotion for all residents. Care is conducted in the patient’s suite and SRRs in-clinic.
