As a Clinical Educator with Vancouver Coastal Health you will :
- Work within a client and family model of care with acute, chronic and palliative populations within the community and within the context of a team-based model of care in collaboration with Primary Care, and members of the Primary Care Network (PCN) Interdisciplinary team.
- Develop and maintain partnerships within integrated community program services and the PCN in accordance with the local PCN delivery model as co-designed by the PCN Partners (local Division of Family Practice, VCH Community of Care and First Nations Partners).
- Provide support and clinical education to PCN teams in multiple locations using specialized theory and knowledge as well as teaching and learning principles and processes.
- Consult, confer and collaborate with the interdisciplinary team members, clinical educators, the Program Manager, and Professional Practice in the promotion of continuous quality improvement related to resolution of clinical issues, educational programming, and in coordination of activities required for progressive clinical learning.
- Take appropriate action with respect to individual and group learning needs.
- Participate in strategic planning as required.
- Assists in achieving interdisciplinary team goals.
- Act as a resource for nurses and other health care providers in the community using consultation, in-services and presentations using specialized knowledge of theory and skills.
Qualifications
Education & Experience
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).Three (3) years recent community health nursing practice, including one (1) year related teaching experience in client education and / or group facilitation, demonstrated knowledge and skills in adult learning and adult education principles, or an equivalent combination of education, training and experience.Completion of the Vancouver Coastal Health Educator Pathway Program is preferred.Valid BC driver’s license and personal vehicle required.Longitudinal care, Chronic Disease Management, and Certified Practice experience as an asset.Knowledge, Skills & Abilities

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
