Responsibilities include but are not limited to : Responsibilities include but are not limited to :

  • Provides clinical operational leadership support to the VBM program focusing on optimizing care delivery on specific cases and on the VBM active patient population as a whole
  • Collaborates with the VBM care team, client, care partners, and the client’s circle of care in the provision of VBM services to optimize care co-ordination and provides advice, recommendations and solutions on the most complex cases
  • Maintains and monitors VBM data collection and reporting on a number of accountability measures.
  • Escalates issues to the Clinical Manager as appropriate
  • Acts as a clinician leader working between VBM clinical team, In-patient behavioural neurology unit (4W), TBSU, LTC, Acute Care and Community Care partners
  • Leads the VBM Team Case Review meetings
  • Monitors and reports weekly on VBM Wait times, number of referrals, maintains the VBM Priority Wait List for Admission to 4W and other data requested by the Clinical Manager
  • Assists clinical manager in VBM program development and preparation of data reports for review and submission
  • Identifies opportunities and takes leadership role to establish linkages with partnering organizations to promote and advance the VBM program and model
  • Coordinates and ensures the effective use of resources to meet client needs.
  • Participates in relevant education and research activities and performs duties consistent with the job classification, as required
  • Oversees the processes for scheduling of VBM appointments to ensure efficient use of team resources and excellent patient, caregiver and care partner experience of care within the VBM model
  • Reviews complex referrals in collaboration with VBM-BSO Intake Office Clinical Navigator
  • Supports complex transitions as needed from VBM to In-patient Behavioural Neurology Unit, LTC, Acute Care and Community settings

Qualifications include but are not limited to :

  • Completion of a Master’s degree program in a healthcare profession
  • Current registration / licensure in good standing as required by a professional body
  • Minimum three (3) years related experience in geriatrics, behavioural neurology, memory and brain health, community-based seniors care and behavioural management
  • Experience / familiarity with Resident Assessment Instrument—Home Care (RAI-HC) and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours
  • Demonstrated expertise and knowledge of working with individuals with various types of dementia, delirium, and those with acute behavioural change
  • Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioural support management approaches
  • Demonstrated excellent clinical-reasoning, critical thinking and problem-solving skills
  • Training in or knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. U-First!, Montessori Approach
  • Effective assessment, intervention, client centered and collaborative care planning skills
  • Experience providing caregiver education, support and counselling relating to caring for a person with responsive behaviours
  • Demonstrated experience working collaboratively with primary care and specialists in the care of older adults with responsive behaviours
  • Ability and willingness to travel within the Toronto LHIN area if needed
  • Proficient computer skills (Microsoft Office Suite) and the use electronic communication devices and software pertaining to risk management and RAI documentation
  • Demonstrated knowledge and experience in providing virtual healthcare, use of OTN using ONE ID, ZOOM
  • Excellent interpersonal, communication, problem solving and organizational skills with demonstrated skills to provide / receive feedback and constructive criticism
  • Demonstrated ability to manage competing demands while setting priorities and meeting deadlines
