Vancouver Coastal Health is looking for a Registered Nurse to join the Home Care Nursing team at Squamish Health Unit in Squamish BC. Apply today to join our team!

As a Community Health Nurse you will :

• Care for clients with acute chronic and palliative care needs.
• Work in partnership with the client and family to establish goals that are safe realistic and reasonable by assessing planning coordinating organizing implementing evaluating and transitioning nursing and other services for the client.
• Assess service eligibility facilitate the development implementation and adaptation of a care plan deliver direct care and provide care management to assist clients to manage their own care and navigate through the various services available within the community settings.
• Work in consultation with other system partners and incorporate current evidence into practice.

ABOUT HOME HEALTH PROGRAMS :

Home Health programs provide a range of health care and support services to people living in the community who have acute chronic palliative or rehabilitative health care needs.

THE CLIENT POPULATION :

Home Health clients typically have on-going health issues and / or complex personal and / or soical issues requiring care planning and coordination of approporiate resources. Clients include frail elderly younger clients with disabilities and adults with chronic illnessess requiring supports to maintain / promote independence.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks.
