This role is unique in its focus on offering emotional support and guidance to patients and families during times of loss and grief. You will play a vital role in respecting and supporting patient wishes through alternative forms of treatment including Medical Assistance in Dying (MAiD) and palliative sedation. Your expertise will be instrumental in providing high-quality patient-centered care making a significant impact on the lives of those you serve.

The Palliative Care team at Hennick Bridgepoint Hospital provides specialized support for individuals whose care needs shift toward comfort function and quality of life. Featuring 24 private rooms we provide specialized care for patients during the advanced stages of terminal illness or chronic disease. The care we provide is guided through collaborative goal-setting ensuring patients are active participants in shaping the support they receive. Our teams approach emphasizes meaningful therapeutic relationships creating space for moments of connection and comfort for both patients and families. We are committed to delivering compassionate support addressing the physical emotional and spiritual needs of our patients and their families.

In this role you will:

• Support collaborative goal-setting by engaging patients in identifying their priorities for comfort function and day-to-day care
• Participate in quality improvement initiatives and incorporate this into the unit workflow
• Provide comprehensive quality patient/family centred health care services
• Provide nursing care in collaboration with the patient significant others and interprofessional team
• Be responsible for all new admissions and the admission assessment
• Provide leadership and contributes to an environment that fosters interprofessional collaborative practice and quality/safe patient care practices
• Establishpatient care plans based on bio-psychosocial and spiritual needs assessment and integrating other health professionals recommendations
• Involve patients and their families in developing the plan of care and in facilitating choice among care options so that care is individualized according to each patients needs
• Support mentor and teach learners
• Perform other duties consistent with the job classification as required

• Current Certificate of Registration in good standing with the College of Nurses of Ontario (CNO)
• Current Basic Cardiac Life Support (BCLS) certification

• Bachelor of Science in Nursing (BScN). Applicants in their final year or near completion will be considered
• Recent completion of a physical assessment course

• Superior assessment and clinical skills
• Demonstrated ability in providing patient-focused care
• Proven ability to make decisions and manage workload in a fast-paced work environment
• Familiarity with:
  • Regulated Health Professions Act
  • Occupational Health and Safety Act WHMIS
• Must have physical strength flexibility and endurance required to perform the role as determined by the needs of the position and based on assessment conducted by Occupational Health Services
