About the Opportunity

RhynoCare is seeking an experienced Personal Support Worker (PSW) to work at a Long Term Care facility in Port Hope . You will be hired and onboarded with RhynoCare as an Independent Contractor.

Responsibilities

• Observe, document, and promptly report changes in residents' physical, emotional or cognitive status to the primary nurse to support ongoing assessment and care planning
• Complete all documentation on time, and in accordance with corporate policies, ensuring that it is both current and accurate.
• Assist resident with activities of daily living (ADLs) including bathing, dressing, grooming, mobility, toileting, and feeding, while promoting dignity and independence.
• Assist in the safe transfer, positioning, and mobility of residents using proper techniques and equipment, and in accordance with facility policies
• Provide care in accordance with the Fixing LongTerm Care Act, 2021 (S.O.2021, c.39, Sch.1), best practices, corporate policies, and procedures.
• Participate in agency risk management program related to resident care.
• Participate in quality improvement program related to resident care.
• Participate in new staff / student mentoring as assigned and promote educational opportunities to enhance their education.
• Participate in departmental, Interdisciplinary Team and resident safety meetings.
• Promote good understanding and teamwork between shifts, registered and unregulated staff
• Adhere to all health and safety procedures, including :
• Use all equipment in accordance with manufacturer's instructions.
• Follow established safety procedures for lock outs.
• Wear proper safety equipment and PPE when necessary.

Requirements

Successful completion of a Ministry of Education approved PSW Training Program (minimum 640 hours) from a Community College or a school approved by the National Association of Career Colleges OR
