We are searching foran Occupational Therapist t o join our Transitional Care Centre located in West Bedford, Nova Scotia.
The Occupational Therapist is integral in supporting the goal of returning to home / community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient / family goals, using effective communication strategies, shared decision-making, and mutual support.
The Occupational Therapist supports a commitment to ongoing quality improvement, continuous exploration and implementation of new approaches, learning, teaching, and mentoring.
The transitional care model supports Nova Scotia Health patients / families who require additional time, services, and support to transition back to home / community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation.

About the Opportunity
- Administers and interprets standardized and non-standardized tests and evaluations to assess occupational therapy related concerns such as level of independence in key life activities required to manage independently, seating and mobility, swallowing and meal texture, pressure reduction and skin integrity, and splinting.
- Focus on maintaining and / or improving the ability to perform key life activities required to maintain independence.
- Develops individualized treatment plans based on clinical records, observation, and referral; confers with interdisciplinary staff in regard to an overall treatment plan.
- Travel between sites, where applicable
- Evaluates progress in relation to occupational therapy treatment provided through use of tests and observation; confers with nursing staff and physicians in regard to effectiveness of treatment
- Prepares progress reports, funding requests, and statistical reports relating to occupational therapy treatment programs maintains treatment records as required
- Participates as a member of the interdisciplinary team; gives evaluation and progress reports. Designs and produces adaptive equipment, utensils, splints or other orthotic devices as needed;
instructs on the use of the equipment.
- Coordinates management of facility equipment; contacts suppliers for service and maintenance of wheelchairs, geri chairs and power wheelchairs
- Provides staff training on ergonomics, seating assessment, skin integrity, equipment and other related topics as required.
About You
In addition to placing high value on continuous improvement, collaboration and accountability, you bring :
- A Bachelors or Master of Science in Occupational Therapy, current registration with the Nova Scotia College of Occupational Therapists
- Knowledge of the principles and practices of occupational therapy, anatomy, physiology, and kinesiology as related to the practice of occupational therapy, the use of adaptive equipment, wheelchairs, positioning chairs, utensils and splints and knowledge of the potential hazards associated with equipment, tools, and materials used in occupational therapy.
- A passion for the healthcare sector and ensuring Nova Scotians have access to quality accommodations, services, and care.

Occupational Therapists will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in central zone, who no longer require hospital care, but need services like occupational therapy before they can safely return home / community.
