Job Summary
The Mental Health Social Worker is responsible and accountable for providing shockingly excellent
patient / customer care as a member of the Mental Health Team.
Primary Responsibilities
- To function as a member of a multi-disciplinary team which provides :
- Assessment, treatment planning and short term counselling of clients with serious mental illness in the Adult Outpatient Mental Health program.
- Responsibilities may include psycho-social work with individuals and in group therapy
- Documentation of clinical assessments, treatment plans and client clinical progress.
- Ability to independently lead client groups, understanding group dynamics and have excellent facilitation skills
- Ability to perform and report to the team on client intake assessments
- Liaison with, and promotion of, good working relationships with team members, the hospital community and community agencies.
- To provide education to the community / hospital.
- Collection of statistical data.
- Committee membership as appropriate.
- Performs other duties consistent with the job classification, as required
Qualifications
- Masters in Social Work (MSW)
- Three to five years experience working with seriously mentally ill clients.
- Ontario College of Social Workers and Social Service Workers (OCSWSSW)
- Advanced counselling and assessment skills with various short-term counselling modalities, including Brief Solution focused model, cognitive behavioural and dialectical behavioural therapy
- Demonstrated ability to work collaboratively in an interprofessional team environment as well as independently.
- Regular attendance is a requirement of the position.
- He / she role models both professional and the organizational core competencies.
- Nonviolent Crisis Intervention Course (CPI) or equivalent
- Ability to de-escalate a crisis situation
- Good Working knowledge of Social Assistance forms
- Understanding of the Mental Health Act
- Understanding of Mental Health Disorders
- Good working knowledge of Social Assistance programs / forms
