Pay Range

Salary Range : $55,000 - $63,000

Reporting to the Client Care and Operations Manager, the Care Team Clinical Lead provides clinical support, education and training to PSWs within dedicated geographical teams. The CTCL will support high quality home support service to clients in the community setting that is compliant with VHA Home HealthCare policy, Ontario legislation and College of Nurses of Ontario (CNO) standards of best practice. The CTCL will promote a culture of interprofessional collaboration among the team and champion a holistic, client and family-centered approach to care.

Key Areas of Accountability

VHA's Care Team Clinical Leads are required to do the following tasks, but not limited to :

Conducting Home Visits (20-25 client visits per week) and develop the client care plan based on the client care needs and authorized service.

Teaching of PSWs with respect to evidence-based practice, including special functions and delegation as per CNO.

Provide ongoing monitoring of the care plan and client safety.

Ensure quality in-service provision, you have excellent assessment skills and understanding of population-based risk.

Working closely with Home and Community Care Support Services and other members of the interprofessional team (PT and OT) as needed to ensure client and PSW safety at point of care.

Conduct and follow up on incident investigations as assigned by the Care Team Supervisor and Client Care & Operations Manager.

participate in clinical focused committees and working groups, as well as liaise with Home and Community Care Support Services (HCCSS) Care Team.

Required Qualification

Training and qualifications as a Registered Practical Nurse (RPN) or Registered Nurse (RN) with CNO registration

Member in good standing of the College of Nurses; membership in the appropriate professional association an asset

Minimum of 2-3 years of experience in community health preferred

Demonstrated efforts to continuously develop and upgrade clinical skills

Demonstrated ability to assess client needs and formulate care plans for clients

Demonstrated ability to write clear and concise reports

Superior interpersonal, verbal, and written communication skills

Organization and self-management skills

Demonstrated ability to work independently and as a member of a collaborative leadership team

Demonstrated ability to problem solve effectively

Knowledge of the social determinants of health and population health (seniors, paediatric, mental health and addictions, LGBTQ+, Indigenous)

Knowledge of the Occupational Health and Safety Act, relevant government legislation including Employment Standards Act and the Ontario Human Rights Code

Computer and technical skills required

Familiarity with GoldCare client management program an asset

Demonstrated ability to monitor and achieve personal objectives and organizational performance targets

Demonstrated commitment to client and family centered care, cultural sensitivity, and diversity

A valid Driver’s License and access to a vehicle are required
