The Primary Care Physician for Vancouver Community Mental Health and Substance Use Acute Home Based Treatment (AHBT) is accountable to the program medical director Vancouver Community Mental Health and Substance Use Services. AHBT is an exciting new city-wide program for clients with acute mental health needs who can safely be managed in their home or housing site with intensive outreach and active medical management. The AHBT Primary Care Physician will be part of an inter-disciplinary team including psychiatrists, nursing, and care coordinators.

The Primary Care Provider is responsible and accountable for the comprehensive assessment of patients including diagnosing diseases, disorders, and conditions including substance use disorders. Treatment planning should include health care management, therapeutic interventions and prescribing medications in accordance with statutory and regulatory standards, limits and conditions, and employer policies and procedures. The physician will provide professional guidance to other health professionals and participate autonomously as a member of the interdisciplinary health teams. An emphasis will be on the needs of clients and their families in the service continuum and ensuring a smooth transition from and to appropriate community health services. This entails engagement with referring clinicians and enabling efficient and safe transfers of care.

Qualifications

Education, Licensing, & Experience :

A medical degree and eligibility for or licensure with the College of Physicians and Surgeons of British Columbia supplemented by experience within a health care organization and a two or more years in independent clinical practice. All VCH physicians must be appointed as medical staff of Vancouver Coastal Health. A clinical faculty appointment with the University of British Columbia Faculty of Medicine is highly encouraged given likely involvement with health profession.

  • Current College of Physicians of BC (CPSBC)- Full; supplemented by experience within a health care organization and a two or more years in independent clinical practice.
  • Canadian College of Family Practitioners (CCFP)- Mandatory
  • Canadian Medical Protective Association (CMPA)- Mandatory
  • A clinical faculty appointment with the University of British Columbia Faculty of Medicine is highly encouraged given likely involvement with health profession

Knowledge & Abilities :

  • Experience in a community health setting preferably working with marginalized populations.
  • Experience in caring for and treating mental illness, addictions, concurrent disorders.
  • Completion of or willingness to complete the BCCSU Provincial Opioid Addiction Treatment Support Program course.
  • A demonstrated ability to work as a member of an inter-professional health care team.
  • A commitment to health care quality improvement with some relevant experience or training.
  • A demonstrated commitment to on-going learning and professional development.
  • Demonstrates knowledge and skills in the following areas :
  • Ability to perform the full scope of Primary Care Physician duties and responsibilities.
  • Awareness of and sensitivity to diverse cultural beliefs and practices.
  • Uses superior communication skills to develop and maintain mutually beneficial relationships with internal and external stakeholders.
  • Ability to apply knowledge of pharmacology, pharmacokinetics, and pharmacodynamics to prescribing and evaluating treatment responses.
  • Ability to communicate and collaborate with patients and families about health findings, diagnosis, treatment, self-care and prognosis.
  • Ability to collaborate, consult with and formally refer clients and patients to physicians and other health professionals when appropriate.
  • Ability to critically assess and evaluate health research literature to determine best practices; ability to introduce education and evidence-based research.
  • Ability to assess and recognize population health trends; ability to plan and implement strategies for population-based prevention and health promotion.
  • Ability to implement and evaluate planned change.
  • Ability to define the specific areas of practice and client population for whom the position is competent to provide health care services.
  • Ability to apply lead leadership skills within a program and team setting.
