As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

Please note that this extended hours position is in the Point of Access, Hospital program.
Staff will work extended hour shifts: 8:30am – 8:30pm, 7 shifts in a two week period; 70 hours per bi‑weekly pay period. Candidates must be flexible to adjust to shifts within the hours of operation. The successful incumbent will rotate through different extended hour shifts as required.
Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s licence and access to a vehicle are necessary.
