Social Worker / Social Service Worker
Position Summary :
As a Social Worker / Social Services Worker, you will work as part of an interdisciplinary team, reporting to the Executive Director / Administrator in the Home. As the Social Worker, you will support / assist residents with managing their overall well-being. You will advocate for the resident as requested, respecting their rights under the Resident Bill of Rights. As a member of the interdisciplinary team, you will support the residents and family members in understanding their plan of care. You will assist to achieve and maintain occupancy targets.
Key Responsibilities :
- Facilitate the admission process in the home and completes all occupancy tracking.
- Facilitate internal room transfers.
- Facilitate discharges from the home.
- Lead the rate reduction application process for residents annually.
- Coordinate family education sessions (virtually and in-person) as requested.
- Act in an advocacy role as appropriate to support residents.
- Provide counselling support to residents and family members.
- Conduct tours of the home to potential resident and family members.
- Coordinate and maintain a good relationship with outside agencies and stakeholders.
- Organize and attend interdisciplinary care conferences including special care conferences.
- Participate in Continuous Quality Improvement initiatives in the home.
- Complete program evaluations and assigned audits.
- Chair of the Ethics Committee in the home, if assigned.
- Participate on the Palliative Care Team / Committee as assigned.
- Prepare all admission packages for the home.
- Act as a resource to the internal Behavioural Supports Ontario team.
- Comply with Plan As guiding principles, all regulatory requirements, and respect the Resident Bill of Rights.
- Meet with Resident Council and Family Council monthly, as requested.
- Other duties as assigned.
Requirements :
Bachelor of Social Work Degree from a recognized university OR Social Services Worker Diploma from a recognized college.Current membership with the Ontario College of Social Workers and Social Service Workers required, and in good standing.Minimum of 2 years experience in the health care sectorBilingualism is an asset.Flexible with working hours to provide evening support as required.Palliative Care experience an asset.Knowledge of Behaviour Supports Ontario an asset.Knowledge of Occupational Health & Safety practices, principles, and legislationExcellent active listening skills.Empathetic, with a passion and focus on the person-centered approach.Strong problem-solving, negotiation and conflict resolution skills, with the ability to foster collaboration, teamwork, and the ability to work with a diverse population.Excellent teamwork and team-building skills.A high degree of resourcefulness, flexibility, and adaptability.Able to effectively communicate verbally and in writing.Politically and culturally sensitive.Committed to staff and resident privacy.Tuberculosis testing within the last two years; Steps 1 & 2; or an X-Ray from the previous five years showing a clear result.Able to provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.Proof of current immunization in accordance with Plan A policy.
