As a progressive and creative organization, CarePartners is proud to be a leader in developing innovative programs. One of our spotlight programs is the eHomeCare Program . This is an innovative provincial program that is unique to home care and includes oversight of the patient through a virtual care platform.
We are looking for energetic and dedicated Personal Support Workers to help patients in our community, with activities of daily living. You will help provide palliative and complex care, and stable pediatric care for children with complex needs. You will provide care to patients in their own homes, working collaboratively with specially trained nurses who will assist you through supportive technology. CarePartners will provide education specific to eHomeCare, as well as a comprehensive, fully paid orientation.
Hours of work will be 11 pm - 7 am overnights Monday-Friday including a rotation of weekends and overnight shifts.

Delivering care and support to patients and their families in their homeProviding quality, integrated care through direct communication between a nurse via laptop / tablet and the eHomeCare Support Worker via smartphone throughout their shiftWhat You Bring
A strong desire and commitment to making a difference in the lives of their patientsPassion for providing holistic, sensitive, individualized care to patients at end of lifeExperience with providing care to patients in their own homes (considered an asset)Ability to work independently and take direction and feedback effectivelyTechnological skills such as smartphones and virtual care delivery platformsExceptional interpersonal skillsAbility to work both independently and as part of a teamAvailability to work non-traditional hours, such as 8 hour night shifts and alternate weekendsA valid driver’s license and vehicleValid certificate as a Personal Support Worker or Internationally Trained Nurse or enrollment in a Nursing programTo protect our patients, we require current vulnerable sector check screening, and a clear background checkNew Ontario PSW grads may be eligible to
receive a $10,000 bonusfrom an approved PSW program in Ontario (on or after April 1, 2023)in exchange for a commitment to work with us in a Full-Time position for 12 months, payable at 6 months and 12 monthsand an additional $10,000 relocation bonus

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
