We are seeking an experienced Community Health Representative to join our community and work with our members and social services team in Scugog, ON on a fixed-term contract of 6 months. The role runs Monday to Thursday, 8 : 30 am to 4 : 30 pm, and Friday 8 : 30 am to 1 : 00 pm, with occasional evening or weekend work as needed.

The Mississaugas of Scugog Island First Nation is a strong, vibrant, and progressive Indigenous community approximately 1.5 hours from the heart of Toronto. Located near the rural community of Port Perry, MSIFN enjoys small‑town living while remaining close to urban areas. As part of the Anishinabek Nation and signatories to the Williams Treaties, MSIFN takes great pride in its Indigenous culture and long history of protecting Aboriginal and Treaty rights.

Job Summary

Under the direction of the Health & Social Services Manager, the Community Health Representative (CHR) supports the entire health team and serves as a key liaison between the team and the community. The CHR delivers health promotion, prevention, and education initiatives, encourages community engagement in health programs, fosters improved health outcomes, and empowers individuals to take an active role in achieving and maintaining optimal health.

Essential Functions

  • Design and deliver community health programming through educational workshops and interactive activities that promote wellness and disease prevention.
  • Foster active community engagement by encouraging participation in health education, awareness campaigns, and promotional events.
  • Coordinate specialized health workshops in collaboration with internal staff and external health professionals on specific health topics or needs.
  • Collaborate with interdisciplinary health teams—both within the organization and with external partners—to ensure culturally appropriate and effective health information delivery.
  • Provide support for individuals and families in understanding and adopting health‑related practices aligned with current initiatives.
  • Serve as a trusted source of health information for the community, guiding residents to reliable resources for health promotion and disease prevention.
  • Promote a safe and healthy environment through awareness programs addressing public health concerns and encouraging healthy living.
  • Advocate for equitable access to health programs and services at both individual and community levels.
  • Develop and implement new health programming, assisting with proposal writing and funding applications when necessary.
  • Educate the community on safe health practices, proper use of health services, and navigating the healthcare system effectively.
  • Disseminate information about available health supports, local providers, services, and referral pathways.
  • Provide guidance on health benefits, including Non‑Insured Health Benefits (NIHB), CINUP, and other relevant programs.
  • Offer preventative health sessions, organize and facilitate workshops focused on early detection, lifestyle changes, and wellness management.
  • Manage program budgets, including preparation, reconciliation, and reporting per organizational and funding requirements.
  • Participate in planning, development, and evaluation of health programming, offering input and consultation on proposed initiatives to ensure alignment with community needs and best practices.

Pay Rate

$29.18–$33.98 per hour (Pay Grade I)

Minimum Qualifications

  • 1 year certificate in a health‑related field.
  • 1 year work‑related experience, or 3 years total work experience, or equivalent training and experience as determined by the hiring committee.
  • Specialization Required

  • Valid driver’s license and $1 million liability insurance.
  • Conditional offer of CPIC acceptable (Vulnerable Sector Search).
  • Current CPR and First Aid certification.
  • Knowledge and respect for Indigenous culture, traditions, teachings, and community dynamics.
  • Strong written and verbal communication skills.
  • Excellent interpersonal skills when working with colleagues, community, and external partners.
  • Detail‑oriented with excellent organizational skills.
  • Ability to analyze problems, recommend and implement solutions.
  • Knowledge of community health principles and practices.
  • Working knowledge of Microsoft Office software suite.
  • Considered an Asset

  • Post‑secondary diploma or degree in a health‑related field.
  • Experience in community health.
  • Work experience with a First Nation or Indigenous organization.
