The Nursing Practice, Education and Community Health Portfolio includes population health, seniors' strategy, ambulatory, complex and palliative care, and contributes to system wide transformative change. As part of this portfolio, the Geriatrics and Supporting Services Program has a specialized focus on geriatric care and enterprise-wide clinical services including skin, wound and ostomy care, and vascular access therapy, providing expert internal and external support to acute and community providers, and is a lead in supporting the senior friendly care strategy across HHS.

Position Summary

The OT / PT functions as an integral member of the interprofessional healthcare team at the JH. They provide comprehensive assessment, treatment planning, and intervention services to patients. Working collaboratively with team members, they apply clinical expertise to maximize patients' functional independence, facilitate recovery, and support safe discharge and community reintegration.

Schedule Work Hours

8-4 - Days, weekends and statutory holidays

Qualifications

1. Graduate of an approved University Graduate level Physiotherapy program (BScPT, MSc. PT) or equivalent

2. Current Registration in good standing with the College of Physiotherapists of Ontario

3. Well developed physiotherapy assessment & treatment skills relevant to clinical setting

4. Demonstrated excellent communication, interpersonal skills and effective discharge planning skills and knowledge of community resources and supports

5. Demonstrated ability to work independently while collaborating within an interdisciplinary healthcare team

6. Participation in continuing education in order to upgrade skills and keep abreast of new developments pertinent to clinical practice area

7. Demonstrated ability to work in a changing environment with high productivity, organizational and flexibility demands with solid clinical reasoning and caseload management skills

8. ADP authorizer status preferred
