About The Role

The Licensed Practical Nurse (LPN) reports to the Director of Care and / or designate. This position is the role model and team leader to an assigned community, and responsible for coordinating care delivery of the persons served. The LPN performs duties and monitors departmental activities in accordance with their scope of practice, established standards, regulations, policies and procedures to maximize individualized and resident-centered care.

Responsibilities

Include but not limited to :

• Be an engaged collaborative team player, developing and maintaining professional relationships that support and strengthen the organizational culture and purpose.
• Assesses residents’ needs; implements and evaluates individualized resident-centered care plans.
• Ensures residents rights are observed and adheres to a holistic philosophy of wellness.
• Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centred care is delivered.
• Assesses and treats all wounds on the unit and acts as a resource to care staff for strategies to prevent skin breakdown. Consult with RN or designate for changes in treatment strategies as needed.

Administers prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
