Social Worker

Schedule : Monday x 4hrs – Wednesday x 4hrs - may be flexibility on this

Reports To : Manager, Clinical Practice

PAY - $45 / hr

Position Summary

Bayshore Medical Personnel is in search of a Social Worker interested in PART TIME work to join a healthcare clinic that provides medical services for all clients of various risk levels. This includes but is not limited to : adult males, adult females, families, pregnant women, 2SLGBTQI+ individuals, minors, persons suffering from a medical condition or disability, addictions, persons with suspected or diagnosed mental illness, and victims of human trafficking.

DUTIES & RESPONSIBILITIES

The responsibilities of this role include but are not limited to :

• Providing personal care assessments.
• Providing individual counselling and therapy.
• Evaluating and monitoring individual progression.
• Maintaining and updating records;
• Making referrals to medical professionals, when appropriate; and,
• Providing awareness about external resources for continuing support.
