PROGRAM OVERVIEW :
The Stronach Regional Cancer Centre (SRCC) is a regional hub for delivery of services and programs for patients and families living in the York Region, North York and South Simcoe County. SRCC is the largest by population in the province and consistently ranks as one of the top Regional Cancer Programs based on provincial performance indicators. SRCC provides a full range of oncology services including diagnosis, treatment, and follow-up within the modalities of medical oncology, radiation oncology, as well as palliative care and end of life care. SRCC is the only Cancer Centre in the Central Region to provide Radiation Oncology.
POSITION OVERVIEW :
The Clinical Associate – General Practitioner in Oncology (GPO) as an important member of the Stronach Regional Cancer Program team provides direct patient care to patients and families within the outpatient Cancer Centre in collaboration with the Oncologists and other team members. Primary responsibility will be the Medical Oncology outpatient population.
The service consists of a variety of oncologic needs including follow up treatment plan reviews, survivorship program support, symptom management and active chemotherapy complications.
DUTIES AND RESPONSIBILITIES :
1. Implement treatment plans and follow-up treatment reviews for patients requiring routine medical oncology / systemic therapy or are under a research program in the cancer clinic.
2. Assessment of follow-up patients and initiation of appropriate tests and investigations when indicated. Altering supportive treatments based on toxicities and ordering systemic therapy when appropriate.
3. Documenting assessments and follow-ups appropriately in the electronic medical record (EMR) routed to the referring Physicians and Primary Care Providers.
4. Initiating referral to home care and / or palliative care services as needed.
5. Facilitating urgent admissions to inpatient Cancer Care (CAN), including documentation in the EMR when appropriate.
6. Completing urgent Medical Oncology / Malignant Hematology outpatient consultations with appropriate and immediate sub-specialty medical oncology supervision / back-up available.
7. Complete relevant oncology diagnostic and therapeutic procedures (i.e. paracentesis, thoracentesis, bone marrow biopsies and lumbar punctures).
8. Providing the point of contact and resource for the inter-professional team regarding goals of care for Medical Oncology / Malignant Hematology patients.
9. Liaising with other hospital specialties to obtain consultation and support for Medical Oncology / Malignant Hematology patients where required.
10. Liaise with SRCC Physicians and Nurse Practitioners for smooth transitions to and from the Outpatient Cancer Clinics.
11. Engaging with the other physician extenders for radiation oncology and hematology to ensure appropriate care of shared malignant hematology and medical oncology patients.
12. As part of the agreed upon time commitment, act as a liaison with community physicians to ensure continuity for patients under their care.
13. Work with the Cancer Care Team to promote the patient–centered model of care and participate in inter-disciplinary rounds, educational events, multidisciplinary case conferences as well as patient / family conferences where appropriate.
14. Providing educational support to learners and the inter-professional team on the unit.
15. As part of the agreed upon time commitment, participate in administrative committees such as Cancer Care Program Committee and Cancer Program Morbidity and Mortality and others as required.

Qualifications :
1. Member in good standing with the Royal College of Family Physicians of Canada (CFPC). Licensed to practice medicine in Ontario.
2. Eligible for appointment to Southlake Regional Health Centre Medical staff.
3. Minimum of two years’ experience in an acute care hospital with experience in cancer will be given strong preference.
4. Knowledge of clinical, administrative, and system-wide issues with respect to cancer.
5. Demonstrated interest in working with cancer patients and families.
6. Demonstrated ability to work both independently and as a member of inter-disciplinary team.
7. Demonstrated ability to maintain positive relationship with other health care team professionals and able to contribute to the development of an environment conductive to excellent patient care.
