About the Opportunity

The Hants Health and Wellness Team (HHWT) is an integrated rural health model aimed at providing a sustainable solution to the provision of primary health care in the communities of the West Hants Network.  The model  includes ongoing development of an integrated chronic disease management and wellness model, which works in a coordinated and synergistic manner with the West Hants Network. The model is grounded in collaboration with other provider and community partners / groups and / services to offer programming and supports to citizens, providing them with the self-management skills and confidence to live well with their chronic condition and to achieve optimal health and wellness.

Reporting to the Health Services Manager, the Wellness Facilitator is the lead support for physical activity programs offered within the community and how to integrate physical activity across programs and service offered by the HHWT.  The individual is responsible for exercise program development, implementation and evaluation and must possess excellent leadership skills.  He / she is outcome-focused, incorporates evidence based best practice, and emphasizes health promotion and disease prevention.  The incumbent is a member of an inter-professional team and collaborates with colleagues to determine team goals and objectives, participates in the evaluation of team effectiveness, solves problems, and shares responsibilities. Excellent interpersonal and written communication skills are required.

About You

We would love to hear from you if you have the following :

  • Bachelor’s degree in physiotherapy from an accredited university
  • Membership with the Nova Scotia College of Physiotherapists
  • Must be eligible for membership with the Canadian Physiotherapy Association
  • One year related experience required working in a Primary Health Care setting / community based team / community setting and experience with individual and group education / facilitation utilizing adult education, self-management supports, and motivational enhancement principles; experience working with people with chronic disease in an exercise program or setting
  • Valid driver's license and access to a reliable car for travel for work
  • ACSM (American College of Sports Medicine) Exercise Specialist Certification preferred
  • Preferred experience in partnership development, stakeholder collaboration, and community development
  • Experience working with people living with a chronic condition
  • Understanding of population health, health promotion strategies, and behaviour change principles
  • In-depth knowledge of local district health authority and community-based services.
  • Demonstrates person-centered / family-focused, diversity relevant approaches to care
  • Well-developed interpersonal, teamwork, oral and written communication skills
  • Demonstrates flexibility, adaptability and willingness to change
  • Demonstrates innovation in program planning, implementation and evaluation, and quality improvement, planning skills
  • Computer skills (MS Office, e-mail and internet)
  • Annual CPR-AED certification and annual WHMIS training and certification
  • Competencies in other languages preferred, French an asset

Hours

  • Permanent part-time / 60 Hours Bi-weekly

  • Compensation and Benefits

    33.97 - $42.89 Hourly
