Catholic Family Services of Peel-Dufferin (CFSPD) is a dynamic, growing family service agency serving all areas of Peel and Dufferin. CFSPD is committed to providing a positive work environment for its employees and excellent services to its clients.

CFSPD is the lead agency to the Safe Centre of Peel - a partnership of 29 social service organizations. The Safe Centre of Peel provides co- located,integratedand coordinated services which addresses the needs of individuals experiencing Intimate Partner.

Position Summary

Reporting to the Program Manager, Safe Centre of Peel, the Client Navigatoris responsibleforacting asthe point of first contact for program participants, potentialclientsand community. The Client Navigator conducts intake, and screening; provides crisis intervention, short-term counseling interventionand advocacy to individuals, families, and makeappropriate referralsto community services. The incumbent alsoestablishesand maintains relationships with on-site and off-site partners, agencyservicepartners and referral sources. The incumbent is also responsible for conducting telephone and in-person outreach activities and completing all appropriate administrative records in a timely manner.

Responsibilities

Triage, Assessment and Case Management

  • Answer client inquiriesregardingservices and support client in providing therapeutic triage and assessment.
  • Ensure all attempts of client contacts are documented.
  • Conduct initial triage / assessment through various avenues such as face-to-face, telephone and email todetermineclient needs, interests, eligibilitycriteriaand next steps.
  • Conduct therapeutic, trauma-informed risk assessment and safety planning
  • Facilitate referrals to on-site and off-site partners, agency programs, other professionals, agencies, services, or community resources appropriate toclientsidentifiedneeds.
  • Respond to telephone, email, and in-person inquiries from clients, prospectiveclientsand community partners.
  • Process andmaintaininternal and external referrals.
  • Assistin the co-facilitation of group programs.
  • Follow up and provide ongoingsupport with high-risk clients
  • Liaise with on-site and off-site partner agencies that have made referrals to the agency
  • Provide telephone support, crisis intervention, safetyplanningand follow-up for clients
  • Provide case management as assigned.

Community Involvement

  • Positively and professionally represent the agency in the community by collaborating with community partners on various projects and initiatives as assigned.
  • Represent the agency at internal and external committees as needed
  • Administration

  • Prepare, compile,submit, andmaintainaccuraterecords, files, statistics, andadditionalrelevant informationin accordance withagency policy, and funding requirements.
  • Keep careful andaccuraterecords andmaintainclient case file of all relevant informationpertaining toassigned caseload.
  • Collect relevant client demographic data.
  • Create, update, andmaintainclient enrollment information in databasein a timely manner
  • Train and supportnew staff,students,and volunteers.
  • Maintain and update agency on-line telephonelogs.
  • Maintain data collection spreadsheets.
  • Other

  • Participate in agency fundraising activities asdeemedappropriateand necessary by Senior Management.
  • Attend required agency activities and meetings.
  • Perform other duties as assigned.
  • Qualifications

  • Bachelor's Degree in Social Work, Community College Diploma or related human service field and two (2) years of relevant work experience preferred.
  • Knowledge of the principles and practices of counseling, social work, and case management.
  • Demonstrate positive interpersonal skills and ability to work in a team environment with co-workers, community providers andstakeholders,and clients.
  • Strong organizational and time management skills, ability to prioritize and meet multiple deadlines.
  • Excellent verbal and written communication skills in English (French as well as other languages an asset).
  • Proficient in Microsoft Office suite and working knowledge of database management.
  • Ability to work independently.
  • A satisfactory Vulnerable Sector Criminal Records check.
  • Valid drivers license, access to a reliable vehicle and relevant insurance.
  • Working conditions

  • Work is performed primarily in a standard office environment.
  • Some travel will berequiredto satellite offices for agency meetings.
  • Evening work isrequired.
  • Physicalrequirements

  • The incumbent will have to spend long hours sitting and using office equipment and computers.The incumbent may also have to do some light lifting of supplies and materials from time to time.
  • The incumbent in this positionfrequentlycommunicates with internal and external stakeholders who require information or need support.Mustbe able to exchangeaccurateinformation in these situations.
