Remote LCSW - Cancer Care CBT & Care Coordination

Daymark Health Inc. seeks a pioneering care-support specialist to lay the foundation of our program and deliver CBT to cancer patients. You will assess member needs, help design care plans, and connect members with community resources throughout their cancer journey.

You will become the Daymark care model expert, building care protocols and maintaining our resources database while documenting in the EHR. Close collaboration with the clinical team and ongoing rounds will be essential.
