Medical Surgical Patient Care Coordinator

Support patient and family centered care at Invermere District Hospital through clinical coordination and resource planning. This relief part-time role works rotating days.

As Patient Care Coordinator, you will coordinate operations and quality patient care across designated clinical areas. The position supports access to appropriate resources and seamless patient movement through the healthcare system. You will liaise with interdisciplinary teams, plan staff education, and contribute to administrative system development.

Key Responsibilities:

• Coordinate care operations within designated clinical areas
• Facilitate patient access to appropriate healthcare resources
• Liaise with interdisciplinary teams for resource utilization
• Review and recommend patient care policies and procedures
• Plan educational needs for staff

Requirements:

• Approved School of Nursing graduation
• Current BCCNM practicing registration
• Advanced clinical specialty preparation
• Recent clinical experience in the assigned environment
• Recent supervisory and teaching experience

Use nursing expertise, interdisciplinary coordination, and quality improvement knowledge to support medical and surgical patient care.
