Help young adults connect goals with community supports as a Casual Transition Coordinator at CHIMO, working days, evenings, and weekends.
CHIMO seeks a Casual Transition Coordinator to work alongside young adults for a period of transition. You will help each person make real progress, step by practical step. Responsibilities include building relationships quickly, connecting young adults with community supports, and supporting group programming when caseloads allow. A related human services diploma or degree and experience with complex needs are required.
Key Responsibilities:
- Work alongside young adults during periods of transition
- Establish effective relationships with each referred young adult
- Support progress on individual goals through practical steps
- Connect young adults with community supports and programming
- Assist with group programming sessions as caseload permits
Requirements:
- Child and Youth Care, Social Work, or related human services credential
- Minimum one year supporting youth or young adults with complex needs
- Ability to remain calm when situations become challenging
- Clear Intervention Record and Police Information checks
- Minimum age 22 with valid Class 5 driver's licence
