Heart Failure Nurse Practitioner

Apply advanced clinical expertise in Fraser Health’s Heart Function Clinic, supporting complex heart failure care through assessment, diagnosis, and treatment planning.

This Nurse Practitioner serves patients with diagnosed or suspected heart failure at Abbotsford Regional Hospital in Abbotsford, B.C. Working with cardiology specialists, nurses, dietitians, pharmacists, and clerks, the NP provides evidence-based outpatient care. The position includes education, medication optimization, consultation, research, leadership, and professional practice.

Key Responsibilities:

• Assess, diagnose, and treat patients within scope
• Order investigations and interpret diagnostic reports
• Develop treatment plans and prescribe medications
• Educate patients and families about diagnoses and prognoses
• Collaborate with physicians and interdisciplinary providers

Requirements:

• Current BCCNM practicing Nurse Practitioner registration
• Recent relevant clinical nursing experience
• Ability to collaborate with patients, families, and professionals
• Ability to critically assess health research literature
• Ability to lead and work within teams
