Coordinate Community Nursing Health Services with CarePartners as a full-time Nurse Manager in Tillsonburg and surrounding areas, using nursing leadership and case management skills.
This Nurse Manager leads CarePartners’ community nursing delivery within its Leadership Team. The position includes coordination, supervision, coaching, care planning, and employee performance monitoring. It supports high quality patient care across community settings and requires collaboration with Ontario Health atHome.
Key Responsibilities:
- Coordinate and supervise Nurses working in the field
- Coach healthcare teams delivering Community Nursing Health Services
- Develop and maintain effective Community Health Services Care Plans
- Monitor employee performance and nursing practice requirements
- Provide active guidance for workplace Health and Safety
Requirements:
- Registered Nurse degree and CNO registration
- Three to five years of related experience
- Knowledge of case management and nursing processes
- Computer skills, including scheduling systems and word processing
- Vehicle, regular travel, communication skills, and required checks
