We are seeking a passionate and experienced Physiotherapist (PT) to join our growing community rehabilitation team. This role is ideal for a Physiotherapist who enjoys the autonomy of managing their own community caseload while being supported by an experienced interdisciplinary team.

This is a flexible, casual opportunity that can be structured as either an employee or independent contractor position, depending on the successful candidate's preference. Whether you are looking to supplement an existing caseload, build your community practice, transition into home care, or simply enjoy the flexibility of accepting referrals that fit your schedule, we would love to hear from you.

You will work closely with clients and their families in the home, completing comprehensive assessments, developing individualized treatment plans, and implementing evidence-based interventions that improve mobility, strength, balance, endurance, and overall function. Many of the individuals we support are living with complex or chronic medical conditions and require Alternate Level of Care (ALC) services.

This is an exciting opportunity to join The Care Company as we continue to build and expand our Physiotherapy program. Beyond delivering exceptional clinical care, there will be opportunities to contribute to new service development, Requests for Proposals (RFPs), quality improvement initiatives, and innovative models of care that can help expand Physiotherapy services across the community.

As our PT program continues to evolve, there will also be mentorship and leadership opportunities for therapists interested in helping shape the future of community Physiotherapy. This includes opportunities to mentor Physiotherapy students during community placements, support the development of future clinicians, assist with onboarding new therapists, and contribute to clinical resources and best practices.

What You'll Do

As a Physiotherapist with The Care Company, you will have the opportunity to make a meaningful difference in clients' lives while enjoying the autonomy and flexibility of community practice. Working collaboratively with an interdisciplinary team, you will:

  • Complete comprehensive in-home physiotherapy assessments within the required referral timeframe, including mobility assessments, balance evaluations, gait analysis, falls risk assessments, range of motion, strength testing, pain assessments, and functional mobility evaluations.
  • Develop individualized, evidence-based treatment plans that maximize mobility, independence, safety, and quality of life.
  • Provide follow-up visits focused on therapeutic exercise, gait training, balance retraining, mobility progression, pain management, post-operative rehabilitation, neurological rehabilitation, cardiorespiratory rehabilitation, and falls prevention.
  • Develop individualized home exercise programs and educate clients and caregivers to promote long-term success outside of treatment sessions.
  • Recommend and train clients in the safe use of mobility aids, gait aids, and assistive devices while collaborating with Occupational Therapists regarding equipment and home safety recommendations.
  • Support clients and families through education, coaching, and practical strategies that promote confidence, independence, and successful aging in place.
  • Collaborate closely with Occupational Therapists, nurses, Personal Support Workers, physicians, care coordinators, case managers, and Ontario Health atHome to support seamless care transitions.
  • Monitor client outcomes and modify treatment plans to ensure meaningful, measurable progress toward each client's rehabilitation goals.
  • Maintain accurate, timely, and clinically sound documentation.
  • Communicate proactively with the office regarding caseload availability, scheduling changes, urgent referrals, and client progress.
  • Independently manage your caseload and schedule while receiving administrative, scheduling, clinical, and technical support from The Care Company.
  • Deliver compassionate, ethical, and evidence-informed care while upholding The Care Company's commitment to clinical excellence.

Beyond Clinical Care

This role offers more than a traditional community caseload. As The Care Company continues to expand its Physiotherapy services, you may also have opportunities to:

  • Help shape and grow our community Physiotherapy program.
  • Contribute to new service development and Requests for Proposals (RFPs).
  • Participate in quality improvement initiatives and innovative models of community rehabilitation.
  • Mentor Physiotherapy students during community placements and help develop the next generation of Physiotherapists.
  • Support the onboarding and mentorship of future Physiotherapists joining the organization.
  • Collaborate with leadership on the development of clinical resources, treatment pathways, and best practices.
  • Grow into future clinical leadership and mentorship opportunities as our rehabilitation program continues to expand.

Scheduling

  • Flexible, casual scheduling with no fixed weekly schedule.
  • Accept referrals based on your availability and geographic preference: The Central Region includes areas such as Thornhill, North York, Markham, Aurora, Newmarket, Richmond Hill, Barrie, Bradford, Alliston, and Simcoe.
  • Initial in-person home assessments are generally expected within 48–72 hours of receiving a referral within your geographic service area.
  • Ability to independently manage scheduling and communicate promptly with the office regarding availability.
  • Travel between client homes is required; access to a reliable vehicle is considered an asset.

Requirements

Qualifications and Experience

  • Registered and in good standing with the College of Physiotherapists of Ontario (CPO).
  • Experience providing home and community care, particularly with HCCSS and/or Ontario Health atHome clients, is strongly preferred.
  • Community physiotherapy experience and/or hospital discharge, rehabilitation, or transitional care experience will also be considered.
  • Demonstrated skill in mobility assessments, falls prevention, gait training, balance retraining, therapeutic exercise prescription, neurological rehabilitation, orthopedic rehabilitation, and post-operative rehabilitation.
  • Experience prescribing mobility aids and collaborating with interdisciplinary teams.
  • Strong understanding of community discharge planning, mobility supports, caregiver education, and promoting aging in place.
  • Strong electronic documentation skills.
  • Experience coordinating with interdisciplinary teams and communicating with physicians and case managers.
  • Ability to work independently in the community and produce clear, timely, clinically appropriate documentation.
  • Valid driver's licence and access to reliable transportation, or the ability to travel efficiently within the designated service area.

Benefits

Compensation

  • $140 per initial assessment, including completion of the physiotherapy assessment, development of an individualized treatment plan, and any required funding applications or recommendations.
  • $75 per in-person follow-up visit.
  • $50 per virtual follow-up visit.
  • $25 per 30 minutes of approved administrative time completed outside of scheduled client visits, including documentation, care coordination, report writing, communication with physicians and interdisciplinary team members, development of home exercise programs, funding applications, equipment recommendations, and other client-related administrative duties.

We believe Physiotherapists should be fully compensated for the work they perform beyond direct client visits, recognizing that excellent community rehabilitation extends well beyond face-to-face treatment time.
