Clinical Care Manager – LPN Full-Time, Salaried Position Position Summary The Clinical Care Manager – LPN provides day-to-day clinical leadership and operational support to the nursing department.

Working closely with the Director of Care, the Clinical Care Manager supports the delivery of safe, resident-centred care and promotes compliance with professional standards, organizational policies and applicable legislation.

This position provides visible leadership throughout the Home, supports nursing staff with clinical decision-making and follow-up, and assists with quality improvement, education, documentation, incident management and resident and family concerns.

The Clinical Care Manager is an experienced Licensed Practical Nurse who demonstrates sound clinical judgment, strong organizational skills and the ability to support and guide an interdisciplinary care team.

Reporting Relationship Reports to the Director of Care.

Key Responsibilities Clinical Leadership Provides day-to-day clinical leadership and support to nursing and care staff.

Acts as a clinical resource for Licensed Practical Nurses, Registered Nurses and Health Care Assistants.

Supports staff in responding to changes in resident condition, complex care needs and clinical concerns.

Promotes resident-centred and evidence-informed care.

Assists with clinical rounds, care conferences and interdisciplinary follow-up.

Supports effective communication between nursing, physicians, pharmacy, allied health and other departments.

Provides clinical coverage and assistance during periods of increased operational need, when appropriate.

Resident Care and Family Support Supports the timely assessment, care planning and evaluation of resident needs.

Reviews and follows up on resident and family concerns in collaboration with the Director of Care.

Promotes respectful and timely communication with residents, families and substitute decision-makers.

Assists with admissions, transfers, discharges and transitions in care.

Supports the management of residents with complex clinical, behavioural or psychosocial needs.

Quality Improvement and Risk Management Participates in quality improvement initiatives, audits and clinical reviews.

Reviews incident reports, medication incidents, falls, skin and wound concerns and other clinical indicators.

Ensures appropriate follow-up, documentation and communication are completed.

Supports infection prevention and control practices and outbreak response.

Assists with the identification of trends, risks and opportunities for improvement.

Supports compliance with licensing requirements, accreditation standards, professional standards and organizational policies.

Staff Support and Education Provides coaching, mentoring and clinical guidance to nursing and care staff.

Identifies staff education needs and supports the delivery of clinical education.

Participates in staff orientation and competency development.

Reinforces policies, procedures, clinical expectations and documentation standards.

Supports a respectful, accountable and collaborative workplace culture.

Brings performance or conduct concerns to the Director of Care for formal follow-up.

Documentation and Clinical Systems Promotes timely, accurate and complete clinical documentation.

Supports staff in the effective use of the electronic health record.

Reviews care plans, assessments, progress notes and clinical records for completeness and accuracy.

Assists with the development, implementation and review of clinical procedures and workflows.

Supports follow-up arising from audits, inspections, complaints and internal reviews.

Operational Support Assists the Director of Care with the coordination of daily nursing operations.

Supports appropriate staffing decisions and the effective assignment of clinical resources.

Participates in leadership meetings, nursing meetings and quality committees.

Assists with clinical supply monitoring and operational planning.

Provides leadership support during the absence of the Director of Care, within the established scope of the position.

Participates in an on-call rotation, where required.

Qualifications Current registration as a Licensed Practical Nurse in good standing with the British Columbia College of Nurses and Midwives.

Minimum of five years of recent nursing experience, preferably in long-term care, seniors care, complex care or a related setting.

Previous leadership, supervisory, clinical lead or care coordination experience is strongly preferred.

Current CPR certification.

Successful completion of a criminal record check and any other required employment screening.

Knowledge of British Columbia long-term care legislation, professional standards and licensing requirements is an asset.

Experience with electronic health records, preferably PointClickCare, is an asset.

Knowledge, Skills and Abilities Strong clinical assessment and decision-making skills.

Demonstrated leadership, coaching and conflict-resolution abilities.

Strong written and verbal communication skills.

Ability to organize competing priorities and respond effectively in a busy care environment.

Ability to build positive relationships with residents, families, staff, physicians and external partners.

Sound judgment, professionalism and discretion when managing confidential information.

Commitment to resident safety, quality improvement and person-centred care.

Ability to work independently while maintaining close communication with the Director of Care.

Proficiency with electronic documentation systems and standard computer applications.

Working Conditions Full-time salaried position.

Primarily weekday hours, with flexibility required based on operational needs.

Requires on-call availability.

Regular presence throughout resident care areas is required.

The position includes both office-based responsibilities and active clinical leadership within the Home.
