Are you an experienced LPN who can assess care needs, coach caregivers and take ownership of daily operations? We are looking for a Care Planner who combines sound clinical judgment with the organization and initiative needed to lead a small home-care team.

Right at Home Canada Fraser Valley is a locally owned home-care company serving older adults and adults with disabilities in Abbotsford, Chilliwack, Mission, Langley and surrounding communities. We help clients live safely and comfortably at home through personalized care and practical support for their families.
Your role
You will oversee care planning and day-to-day office operations, working closely with our office team. You will ensure caregivers are prepared for their assignments, families receive timely follow-up, and staffing capacity supports reliable service and business growth. This is an in-person role involving office work and visits to client homes.

Schedule and travel
The role is based in our Abbotsford office, with regular client and community visits across the Fraser Valley. Some flexibility for client needs and participation in a shared on-call rotation will be required.
Care planning and service quality
• Conduct in-home assessments and create clear, individualized care plans that reflect client needs, preferences, routines and safety risks.
• Match caregivers to clients based on demonstrated skills, experience, availability and suitability.
• Provide practical orientation in the client’s home. Demonstrate care tasks, observe the caregiver performing them and arrange further coaching when needed.
• Complete supervisory visits and follow up with clients, families and caregivers. Resolve concerns, monitor changes and update care plans promptly.
• Maintain accurate client records and communicate changes to the care team. Recognize concerns requiring referral or escalation and practise within LPN scope and individual competence.
Office operations and staffing
• Monitor hiring needs by location, availability and required skills; help recruit, interview and assess caregivers and oversee their onboarding.
• Guide the administrative team, set priorities and ensure assigned work is completed.
• Support urgent coverage decisions and caregiver continuity.
• Track weekly scheduled and delivered care hours, cancellations, unfilled shifts and caregiver capacity. Identify trends and develop practical actions to improve service, retain clients and grow hours.
Client relationships and business growth
• Respond promptly to inquiries, explain care options and follow through from initial contact to assessment and service start.
• Ensure client onboarding documents are complete and the care team is ready before services begin.
• Build referral relationships with health professionals and community organizations, and participate in relevant outreach activities.
• Provide the owner with a concise weekly update on care hours, staffing needs, client concerns, inquiry follow-up and actions underway.
• Delivers one-on-one and group presentations (e.g., Lunch & Learns) to promote services.
• Identifies and participates in relevant trade shows, wellness fairs, and senior-focused events.
What you bring
• Current practising LPN registration or licensure with BCCNM, in good standing.
• Experience caring for older adults and people with complex support needs.
• Home care, community care, dementia care or care coordination experience is preferred.
• Leadership or supervisory experience, including coaching staff, assessing skills and addressing performance concerns respectfully.
• Strong judgment and follow-through: you can prioritize a busy day, identify what needs attention and act without repeated prompting.
• Clear written and verbal communication and confidence working with clients, families, caregivers and referral partners.
• Comfort reviewing service-hour data, identifying operational gaps and turning findings into an action plan.
• Proficiency with Microsoft Office and electronic care records. Experience with AlayaCare or similar CRM system is an asset.
• A valid driver’s licence, reliable vehicle for local travel, current CPR and first aid certification, and a current criminal record check.
