ethree Consulting is proud to partner with the Newfoundland and Labrador Medical Association in their search for a Practice Facilitator with The Family Practice Renewal Program (FPRP), a joint initiative of the NLMA and the Department of Health and Community Services (DHCS). The NLMA functions as the employer for staff of FPRP.

The Family Practice Renewal Program (FPRP) aims to advance family practice for better health. We work closely with family medicine physicians, policymakers, and community organizations to identify opportunities for change and work towards solutions within primary care. Our vision is to see family physicians, primary health care providers and patients working together for an effective and sustainable system.

What You’ll Do

Reporting to and assisting the Program Manager, Practice Improvement, the Practice Facilitator is instrumental in supporting primary care practices throughout the province to optimize their practice environments. This role involves providing hands-on, evidence-based support as part of a dynamic provincial program team dedicated to assisting family physicians and their teams. The facilitator addresses common challenges and streamlines processes to elevate practice effectiveness and assist practices to implement new initiatives and ideas into their environments.

Collaboratively, the Practice Facilitator will partner with teams across all regions, engaging with local clinics to foster improvement initiatives. They will be equipped with a suite of practical tools and resources. In this capacity, the Practice Facilitator will guide physicians and their staff through the planning, testing, and implementation of tailored improvements, directly addressing the unique needs of their practice environments and patient populations.

Success in this role relies on strong collaboration with family physicians and their teams to understand their challenges and identify opportunities to make meaningful changes designed to improve patient outcomes. You are a self-starter, comfortable with ambiguity as we grow this program together, leading to a stronger, more effective health care system for the province.

Responsibilities

  • Relationship Building & Engagement : Developing strong, professional relationships with clinic teams, physician leaders / champions, and stakeholders, engaging them to understand their goals for better patient care, better work experience, and improved quality. Working collaboratively with program leaders and physician champions to inspire teams and leverage diverse strengths and structured approaches to change to achieve clinic goals.
  • Practice Capacity Building : Building the capacity of primary care practices to deliver integrated Patient’s Medical / Health Home models, tailoring support to varying levels of readiness, practice behaviours, resources, knowledge and skills. Introducing provincial / program quality improvement opportunities, programs and resources to clinic teams.
  • Practice Assessment & Improvement Planning
  • Facilitating the implementation of Patients Medical Home / Health Home practice assessments to identify gaps, opportunities, and strengths, helping teams prioritize improvement efforts. Supporting learning through events, workshops and webinars.
  • Continuous Quality Improvement : Strengthening clinic teams’ quality improvement capabilities using the Model for Improvement, supporting implementation of quality improvement measures and evaluation. Helping clinic teams share results and lessons learned to accelerate improvement across practices and sustain long‑term progress.
  • Resource & Tool Development : Developing tools and resources that support interactive learning initiatives, both virtually and in‑person.
  • Program Support : Completing other duties as required to support the priorities and initiatives, within the program.

What You’ll Bring

  • A Bachelor’s degree in Public Health, Health Administration, Nursing, Social Work or Business or another related field.
  • Minimum 3-5 years' experience in the healthcare sector (two years’ experience working in a primary care setting is preferred, but not mandatory).
  • Experience with project management, change management and / or quality improvement.
  • Excellent written, oral, and electronic communication skills in English; able to convey complex information clearly to varied audiences.
  • Ability to build professional, collaborative relationships with diverse stakeholders, including physicians, government partners, regulatory bodies, and national associations.
  • Self-starter, able to take initiative and anticipate organizational needs in an ever-changing environment.
  • Thrives in a team environment, demonstrating professionalism, trust, respect, and integrity in handling sensitive information.
  • Highly organized with excellent time management skills; attention to detail.
  • Enthusiasm for continuous learning and improvement and ability to facilitate and build capacity of practice team members and physicians to reach the outcomes set by the organization.
  • Ability to use evidence, data, and experience to facilitate decision-making.
  • Objective and able to represent different perspectives and manage critical conversations respectfully.
  • Experience with the provincial Electronic Medical Record (EMR) would be an asset.
  • A valid driver's license with access to a reliable vehicle for travel is required.
  • Proficient in use of full suite of Microsoft programs and platforms, including cloud-based file systems, such as OneDrive and SharePoint.

    This position is a 12-month contract, with the possibility for review and / or extension at the end of the contract.

  • The salary range for this position is $70,200 to $95,000, with placement within this range commensurate with education and experience.
  • Annual leave starting at 25 days per year.

  • This position is based at NLMA House in St. John’s, with an option to work in-office, from home, or a combination. You must reside Newfoundland and Labrador (if residing outside the capital, willing to travel to St. John’s during office hours occasionally as required, at own expense).
  • This role involves travel throughout the province up to 20-30% of your time, to advance your work with physicians and clinic teams. Travel that is part of your core responsibilities will be covered by the organization.
  • Regular work hours are 9 to 5 Monday to Friday, with some flexibility to work outside these hours. Occasional evening and weekend hours may be required.
