Maintain accountability for the nursing and personal care of residents within the scope of training and licensureAssume responsibility for notifying family / responsible party of any significant change in a resident’s conditionMaintains resident records and required documentation in accordance with policies and procedures and legislative requirementsAdministers medications as prescribed in accordance with pharmacy policies & procedures and observes changes in condition and reports them to the Director of WellnessSeeks direction from the Director of Wellness when resident need exceeds experience or scope of practiceExercises sound nursing judgment by reporting any change in resident’s status in a timely manner to the Director of Wellness as necessary, and takes appropriate actionMonitors the safekeeping of drugs and narcotics, and ensures narcotic count is correctReports incidents / near misses, illness or problems with residents to the Director of Wellness / General ManagerReports all transfers to hospital or deaths immediately to the Director of Wellness / General ManagerComplete and maintain routine or adhoc clinical assessment of residents as assigned by Director of Wellness.Assist in care plan developmentGreet all visitors and family members in a positive and supportive manner requesting if they need assistance of any kindPromotes and maintains positive communication and relationships among team membersDemonstrates personal attributes and communication skills that foster customer satisfaction at all times and monitors team member performance in relation to customer serviceFosters positive interactions with residents, families and visitorsFacilitates problem solving of resident / family concerns or complaints that arise and ensures complaints are communicated to the Director of Wellness / General ManagerAssists with the orientation of new residents to help facilitate a smooth transition to the homeAdheres to organizations policies, procedures, practicesFacilitates calm reaction and resolution to problems that arise or emergency situationsParticipates in Quality Improvement activities as assigned by Director of WellnessAssume responsibility for replacing team members as requiredContinually assesses his / her own nursing practice and learning needsParticipates in and attends in-service programs provided by the community as well as conferences, seminars, and workshopsWorks with the resident, family members, and interprofessional team to systematically collect data for the health history on admission, readmission, and change in status, and provides ongoing assessment and appropriate documentationAssisted Living Experience is an assetDemonstrated problem-solving abilities to lead unregulated healthcare workers and provide effective careWell organized and goal oriented to be able to manage the resident home area and assign dutiesMust possess strong communication skills to communicate with residents, physicians, and other team membersMust be detail-oriented in keeping track of multiple residents’ vitals, and medication schedules.Proficiency with WORD, email, internet use and computerized documentation.Performs other duties as assignedWhat you need to know :
