ABOUT THE ROLE

Under the direct supervision of the Manager or Supervisor, the Discharge Planner provides support, service planning and delivers substance use treatment and education programming. Key responsibilities include program development and implementation, client education, and the creation and maintenance of community linkages as related to short-term discharge and treatment planning.

WHAT YOU BRING

Education / Experience :

  • Baccalaureate degree in Social Work (BSW or HBSW) or related discipline
  • Current registration with a professional college or professional regulatory body
  • Specialized training and experience as appropriate to the client population

Skills / Abilities :

  • Thorough understanding of all aspects of substance abuse and / or problem gambling
  • Ability to manage a clinical practice in a health care setting
  • Ability to complete analysis, using clinical knowledge, skill and judgement to synthesize assessment information
  • Ability to communicate effectively and concisely, both orally and in writing
  • Ability to work independently, interdependently, and participate as a contributing member of a variety of teams within the workplace, community and ministries / agencies
  • Strong time management, analytical / critical thinking, problem solving and decision making skills
  • Ability to use independent judgment and maintain confidentiality of records, actions, and information
  • Demonstrated regular attendance at work
  • Conditions of Employment :

  • This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
  • Offers of employment made to external candidates are conditional upon providing proof of a complete primary series of Health Canada approved mRNA COVID-19 vaccine or proof of valid medical exemption, prior to the offered effective date. If proof of vaccination or valid medical exemption is not provided, the offer of employment is rescinded.
