Job Summary :

The Occupational Therapist (OT) is responsible for providing therapy services to individuals in the community within the framework of VON policies, procedures, standards, and quality and risk management. The OT collaborates with families, caregivers, healthcare professionals, and community services to deliver client-centered, evidence-based therapy services.

Key Responsibilities :

• Assess, plan, evaluate progress, and document patient care plans based on occupational therapy standards and evidence-based practices.
• Evaluate patients' ability to function safely in their home and community environments, including ADLs and equipment needs.
• Assess cognitive, mental health, social, spiritual, and physical factors impacting independence.
• Develop client-centered care plans collaboratively with patients, families, and healthcare teams.
• Provide education and recommendations to patients and families regarding therapy goals.
• Assist with funding applications for equipment and home modifications.
• Monitor patient outcomes and adjust care plans accordingly.
• Document patient progress and care plans in electronic health records as per guidelines.
• Communicate effectively with funding agencies and team members regarding patient status.
• Manage caseloads and respond to changes in patient needs.
