Job Overview
Join our Home Care Nursing team at Squamish Health Unit in Squamish, BC. As a Community Health Nurse you will care for clients with acute, chronic, and palliative care needs, work in partnership with clients and family, coordinate services, and incorporate current evidence into practice.
Responsibilities
- Care for clients with acute, chronic, and palliative care needs.
- Work in partnership with the client and family to establish safe, realistic goals by assessing, planning, coordinating, implementing, evaluating, and transitioning nursing and other services.
- Assess service eligibility, facilitate development and implementation of a care plan, deliver direct care, and provide care management to help clients navigate community services.
- Consult with other system partners and incorporate current evidence into practice.
Education & Experience
Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).At least one year of recent related clinical experience with complex clients / patients with chronic conditions and / or functional mobility issues, including care and transition planning.Valid BC Driver’s License required for local travel.Knowledge, Skills & Abilities
Community health nursing theory and practice within a client‑ and family‑centred model of care.Client‑focused, trauma‑informed, harm reduction, and culturally safe assessment and intervention.Ability to assess safety and risk in home environments and integrate data for problem solving.Conflict resolution and reconciliation approaches for psychosocial and behavioural issues.Independent management, prioritisation, and continued professional development.Adapting to new or unexpected events with client‑focused care and advocacy.Interdisciplinary teamwork skills.Documentation in electronic medical records, including use of related software.Commitment to confidentiality and privacy.Effective communication across verbal, nonverbal, written, electronic, and social media.Facilitation and coaching of adult education principles.Knowledge of provincial acts, regulations, and program policies.Knowledge of acute, chronic, palliative, mental health, substance use, and supportive self‑management.Knowledge of community resources and quality improvement methodologies.Planning, organising, scheduling, and prioritising work.CPR techniques and use of nursing equipment.Understanding of BCCNM standards of practice and guidelines.Physical ability to perform job duties.Work Hours and Conditions
The hours of work, including days off and work area, may be subject to change in accordance with operational requirements and the applicable collective agreement and statutes.
Health and Safety
