Team Leader, Projet Logement Montréal

As a team lead under the supervision of the Partnerships Coordinator, you will play a key role in guiding and supporting the PLM intervention team. You will ensure service quality and efficiency while actively contributing to the program’s development.

Your Responsibilities :

• Train and support the team, ensuring goals are met and guidelines are followed.
• Assist intervention workers in complex and urgent situations (crises, deaths, etc.).
• Collaborate with external partners and participate in coordination meetings.
• Work directly with beneficiaries : welcoming, assessing, planning actions, and providing personalized follow-up.
• Facilitate access to housing, health, and psychosocial services to enhance client autonomy.
• Oversee the planning and organization of activities stemming from PLM according to established procedures and the values of Welcome Hall Mission (WHM).
• Continuously assess and ensure that client needs are met, and contribute to development efforts to address these needs meaningfully.
• Attend various meetings (team leads with clinical coordination, team leads and PLM coordination) and ensure relevant information is shared with the PLM-WHM team.
• Collaborate with coordination to ensure smooth operations and synergy within the team.
• Manage team case files and renew OMHM documentation.
• Participate in developing and implementing solutions to address challenges within the team.
• Perform other related tasks.
