Position Summary :
The Nurse Practitioner will work as an integral part of our interdisciplinary team to provide comprehensive primary health care services.
As an independent professional, the Nurse Practitioner will work to full scope of practice to contribute to clients’ health providing assessment, prevention, education, rehabilitation, and therapeutic support services consistent with the standards of care outlined by the College of Nurses.
We are seeking a Nurse Practitioner who is committed to community-based, client-centered care, and passionate about working collaboratively to address the social determinants of health, to reduce barriers and improve access to health care for the clients across all sites.
Responsibilities Include :
Direct Client Care :
- Provide comprehensive primary health care including assessments, diagnosis, treatment plans, follow up, referral, education and referral within the scope of practice of a registered nurse in extended class
- Maintain accurate client visits and findings on current record system
- Coordinate client care within FHC and / or HATP and with other health care agencies and specialists
- Determine the need for and orders and interpret screening and diagnostic laboratory tests, chest and limb x-rays, diagnostic ultrasounds etc.
- Counsel and provide information and follow up on abnormal results.
- Prescribe designated drugs as specified by the CNO
- Participate in Quality Improvement Programs.
Health Promotion and Education :
- Implement health teaching and promotion with clients as part of clinical assessments and client care
- Participate in identification of priority groups or targeted populations for special health promotion and community development initiatives
- Participate in the development and delivery of group programs or special presentations
- Activities are to be consistent with the mission and goals of FHC and / or HATP, working cooperatively with partners and with other health practitioners in the community
Organizational Responsibilities :
- Participate in Quality Improvement, Audits, Accreditation and other relevant organizational committees and working groups and team meetings
- Consult with the appropriate team physician re clients following the consultative guidelines of the College of Nurses of Ontario
- Participate in the planning, implementation and evaluation of goals which are developed yearly.
- Plan, participate and assist identifying priority populations and implementing effective services consistent with FHC and HATP’s vision and values
- Develop and maintain knowledge of the Flemingdon Health Centre communities and have working knowledge of community services and resources to make appropriate referrals
- Comply with all relevant FHC and / or HATP legislation policies including privacy legislation
- Support teaching of nursing and medical students as needed and support volunteer placement programs as required
- Participate in continuing education activities and assist in the in-service training of other employees
Qualifications, Skills, and Proficiencies :
- Registered Nurse with the College of Nurses of Ontario in the Extended Class.
- Baccalaureate in Nursing and graduate of recognized Educational Program for Primary Care Nurse Practitioners
- 2-3 years of experience in primary care or community nursing will be an asset.
- An ability to work as a member of an interdisciplinary health care team, responsible and accountable for own practice
- Demonstrated experience in chronic disease management and prevention, and a good understanding of the role of health promotion
- Experience working with marginalized communities and complex clients
- Commitment to anti-racism and anti-oppression practices
- Excellent oral and written communication skills
- Ability to work flexible hours, including evenings and weekends
- Ability to speak other languages, preferably Spanish, or Arabic is an asset
- Knowledge of EMR System an asset (Practice Solutions)
- Vaccines (COVID-19 and others) are a requirement of the job pursuant to the Ontario Human Rights Code
