POSITION SUMMARY :
The Personal Support Workers – Integrated Comprehensive Care (PSW-ICCP) enhance the quality of life of SJHC clients by providing quality and compassionate care in the form of personal care assistance, medication reminders, physical and cognitive support and home keeping assistance. PSW-ICCP significantly assists in enabling individuals to live in a safe and healthy manner after being discharged from hospital.
The PSW-ICCP works cooperatively with the ICCP Sr. Manager, Supervisor, Program Assistants and other care givers in providing quality client-centered care, and in the effective execution of daily tasks. PSW-ICCP adheres to daily care schedules and overall care plans.
ICCP clients have recently been discharged from many different pathways and different diagnoses.These clients required more support in the daily management of their own care needs and daily living activities for a short period of time. In some instances, the PSW may have enhanced learning and Skills Transfer opportunities in order to provide care to a certain client population.

- Assisting clients in daily activities which maintain adequate levels of mental / physical health and safety, in keeping with their individual care planPerforming safety wellness checksMedication reminders / medication administrationAssisting in the client’s use of medical devicesLiving area visual scansEngaging clients in positive / social conversations Encouraging clients to direct and / or participate in their own careAssisting clients with general mobility needs (walking, movement, positioning, fine motor movements etc.) in and around their homeEncouraging participation in social and / or recreational activitiesEncouraging positive life skillsAssisting with basic range of motion exercises where required by the care planMay be trained for additional work tasks within scope of practice (eg peritoneal dialysis, hemodialysis)Demonstrating sensitivity and respect to clients who may be experiencing cognitive, physical, and / or mental health challengesProviding emotional and social support to both clients and their family membersDemonstrating a positive attitude and approach towards clients as unique individuals

PSW Certificate from an accredited training institution, required1-3 years PSW experience, preferredMember of the PSW Registry, preferred (may be required under some contracts)Current First Aid and / or CPR Certificate, preferredTraining in gentle persuasive / non-violent crisis prevention, an assetTraining and previous work experience in Medication Reminders, preferredPrevious experience working in an Integrated Comprehensive Care environment, as asset
