Job OpeningaLocation : Expected Start Date : Full Time : Permanent : Posting End Date : Job Description : Diabetic Foot Nurse - Registered Nurse - EMAPS E EquivalentExisting Grant-Funded Continuing Full-Time 35.00 hours / week Monday to Friday 8 : 30 am to 4 : 30 pm - travel requiredSalary Range : $41.01- $50.30 per hourProposed Start Date : June 10, 2024Trial / Probation Period : 840.00 work hours For more information, please contact : Leanne Rajotte204-789-3533 or @ : - Performs diabetic foot assessments and care, lower extremity ulcer care and initiates and / or recommends appropriate anti-microbial therapy (wound care) in the event that a patient requires urgent intervention using management strategies as outlined in the current Manitoba Diabetes Care Recommendations and the current Canadian Diabetes Association Clinical Practice Guidelines. - Performs basic assessments of blood flow (ABPI) using a blood pressure cuff and Doppler ultrasound probe and may direct further management. - Establishes appropriate care plans for individual patients in consultation with HSC Diabetic Footcare clinic specialists (I.D., vascular and orthopedic, orthotic department, amputee rehab). - Responsible for maintaining necessary clinical equipment and adhering to existing infection control guidelines.- Involved in educating all health care providers, family members, patients and community members about the lower limb complications of diabetes. - Participates with the primary health care team in education strategies for secondary prevention including glycemic control and smoking cessation. - Attends community events such as evening cooking classes organized by the ADI worker- give a talk on foot care. - Handout pamphlets on Diabetes and Footcare, educational material for patients.- Responsible to be familiar with the resources available in the tertiary care centres of Winnipeg and able to interact directly from the northern community with specialists to facilitate the delivery of care to the patient.- Attends the Diabetic Foot and Complicated Wound Clinic at the Health Sciences Centre as a liaison to facilitate continuity of care for the First Nations individuals from program communities. - Works in conjunction with the community-based primary healthcare team to ensure optimal management and follow-up. Refers patients to retinal screen program. Refers to orthotics and community home care for ongoing foot care.- Education with visiting medical students and other health professionals.- Expectations to represent Ongomiizwin Health Services with community education about Foot Health and Diabetes and this may take the form of workshops, speaking engagements or exhibit table at workshops and conferences. - Participates in clinical research and program evaluation with clinicians of the Health Sciences Centre and the Department of Community Health Sciences, University of Manitoba. - Responsible for maintaining a patient database with appropriate parameters for monitoring patient outcomes and program activities. Communities shall be involved in clinical research and program evaluation in a manner consistent with existing standards of participatory research.- Develops and maintains clearly written procedures, indexes, logs and records for activities within his / her area of responsibility. - Maintains appropriate file systems and records with guides and indexes, including computer records, back-up files, etc. - Performs other related duties as necessary or as assigned. - Plans / prepares for community travel / visits. Trip reports. Writes quarterly and annual statistical reports on the DFNC program. Maintains footcare equipment via ordering supplies and updated equipment. - Ensuring the NIC is aware of any clients who require any specific intervention related to the current foot care assessment i.e pt required urgent / emergent specialist consult related to a foot : MINIMUM FORMAL EDUCATION / TRAINING REQUIRED : - Registered Nurse with current CRNM registration without restrictions required.- Foot care Nursing course is mandatory. - Sensitivity and knowledge of Indigenous language and culture and asset. - Master's or Bachelor's degree in nursing or related discipline desirable. Knowledge of Diabetes is required. EXPERIENCE : - Experience performing Diabetic assessments preferred.- An acceptable equivalent combination of education and experience may be considered.
