Reporting to the Patient Flow Director, the Patient Flow Coordinator plays a key role between Providence Care's staff, physicians and external partners in ensuring seamless, quality and efficient patient flow activities. The Coordinator responds to inquiries from key stakeholders directly and / or indirectly and assists the Manager by researching and providing requested data or background information, as requested. The Coordinator problem solves barriers to transitions in care by working closely with the Access to Care and Transition and the clinical program teams to identify solutions. The Coordinator completes daily, weekly and monthly summaries provided to relevant leaders in the hospital and is fully informed of issues and challenges that require action. Utilizing sound clinical judgment and strong assessment skills, the Access and Flow Specialist screens and evaluates potential candidates for admission, collaborating with patients, families, care teams, and community organizations to coordinate person-centered transitions. The role requires adaptability and responsiveness to evolving system pressures and patient needs, often supporting flow initiatives as they evolve.
Skills And Qualifications Required
- Baccalaureate degree in Nursing, preferred or an equivalent combination of education and experience
- Master's degree in health studies from a recognized educational institution, preferred
- Current, unrestricted registration with the College of Nurses of Ontario
- Two years of experience in providing clinical practice in a hospital setting with the relevant population. Intake and discharge planning functions preferred
- Experience with electronic patient record systems

Leads Bed Utilization Meetings and other key meetings, as assigned, to facilitate communication and coordination related to patient flowReviews Daily Dashboard and clarifies data quality issuesProblem solves barriers to transitions in care at admission, transfer and dischargeProvides daily and weekly summaries to Providence Care leadershipReview trends, variances and reasons for variances for admissions, transfers and dischargesCollaborates with acute care and community partners to identify individuals who may benefit from Providence Care’s programs and servicesTriages inpatient referrals and conducts timely assessments using a comprehensive, evidence-informed framework to support admission decisions and patient flowProvides clear clinical recommendations to inform Access to Care decisions related to admission and dischargeCoordinates with internal and external stakeholders regarding transfer logistics, medical stability, documentation requirements, repatriations, and service availabilitySupports the identification and implementation of improvement strategies that remove barriers in access to care, and optimize the experience of patients and familiesMaintains awareness of real-time system pressures and actively problem-solves barriers to admissions, discharges, and internal transitionsCollaborates with interprofessional teams, patients, families, and community partners to support safe, person-centered discharges and transitions across care settingsWhy Work At Providence Care Hospital
