Transitional Case Manager, Queens Ave.
Direct Service - (OPSEU)
Full-Time, Contract (Ending : June 30, 2026)

Our Case Management Program embodies this belief by supporting individuals transitioning from hospital to live as independently as possible, while working collaboratively with a Transitional Case Manager to build their support systems. As a Transitional Case Manager, you’ll also focus on assisting clients with connecting to their community and teaching life skills to help them to achieve their recovery goals.
Successful candidates must have a Police Vulnerable Sector Check (PVSC) completed in the last six months. Interested applicants who reside in the City of London and are in need of a PVSC are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 3 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.

WHAT WE'RE OFFERING :
What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ( $30.01 per hour for Level 1 – Direct Service, Regulated) :Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)For internal applicants, grid level placement will occur as per the Collective AgreementWhen you'll work - 70 hours over two-week period; 8 : 30 am to 4 : 30 pm, Monday to Friday, with one unpaid hour for lunch. Assess client’s needs for skill development and supportAssess the adequacy of resources / supports currently available to the clientAssess barriers to successful community living (e.g. treatment options, transportation, finances, etc.)Assist client to identify their personal goals and preferences and utilize standardized tool (GAIN, OCAN, etc.)Design rehabilitation plans based on individual needs, goals and preferencesAssist the individual to navigate systems and connect to appropriate resourcesHelp clients work towards their vision of a positive personal futureWork in partnership with the client in all aspects of planningEstablish an intervention plan which includes expectations and responsibilities of the client, TCM, other supports (e.g. family) and resources, as well as the frequency and nature of interventionsProvide skill teaching and / or support client to obtain the necessary skills training from other sourcesProvide needed information and education about addiction and mental health, coping skills, services, resources, and consumer initiatives / supportsAssist client to identify and connect with personal / natural supportsMeet with client regularly to work on specific goals until other permanent supports are in placeDemonstrate knowledge of range of crisis prevention and intervention approaches including : crisis prevention strategies, crisis planning, crisis intervention, risk assessment, relapse prevention strategies, crisis services and hospital servicesConduct ongoing assessments of client’s progress towards recovery and the need for ongoing intensive case managementEnsure important information is communicated to all relevant stakeholders and documented in the recordWHAT YOU'LL NEED TO APPLY :

We require the following qualifications :
Diploma or degree in a health, social service or a related discipline ( primary proof of credentials will be required during the hiring process i.e. original degree / diploma or official transcript)Valid driver’s licence and access to a vehicle with adequate insurance to carry out job dutiesAbility to attend work regularlyWe prefer the following qualifications :
