Job Description

Reporting to the Patient Care Manager, the Registered Social Worker will be experienced in working in a fast-paced, interprofessional, inpatient environment providing assessment, case management, advocacy, patient education, and community linkages to individuals with serious mental illness.

The incumbent will be expected to provide service to the Inpatient Mental Health Program where they would complete psychosocial assessments and provide interventions in response to the unique and complex needs of each client to ensure comprehensive discharge planning. The Social Worker will plan, organize and implement care services as part of an inter-professional team using the principles of Psychosocial Rehabilitation within a Recovery Model of service. Family, individual and group skills are required as well as the ability to link with other service providers in the Hospital and community to ensure the client is well supported upon discharge. The Social Worker will focus on reintegration of clients into community-based treatment services. The overall goal of the service is to enhance the functioning of individuals with mental illness and their families.

The Social Worker is also expected to contribute to program development and participate in research activities, and organize and collaborate in the planning and implementation of social work best practices within interdisciplinary team environment.

What must you have?

  • Master’s degree in social work from an accredited University.
  • Member of the Ontario College of Social Workers and Social Service Workers.
  • Minimum of 3 years of mental health experience.
  • Experience working in psychiatric / mental health and working with individuals from diverse ethno cultural backgrounds.

What else do you bring?

  • Working knowledge of relevant legislation including the Mental Health Act and the Criminal Code of Canada; Health Care Consent Act; Substitute Decisions Act and Child & Family Services Act.
  • Demonstrated knowledge of Canada Pension Plan Disability, Ontario Disability Support Programs, Powers of Attorney, and Alternate Level of Care.
  • Demonstrated knowledge and skills in the area of Psycho-Social assessments of individuals and families.
  • A strong desire and capacity to promote professional practice and excellence in evidence-based, patient centered care.
  • Well-developed counseling skills in individual, family and group modalities.
  • Ability to work independently and in a team setting.
  • Demonstrated success with complex case management and discharge planning.
  • Individual, couple, family and group therapy experience.
  • Superior skills required in communication, advocacy on behalf of clients, collaboration with inter-professional team, consultation with professional colleagues in the Hospital and the community.
  • Strong interpersonal skills to function as a productive member of the interdisciplinary team.
  • Demonstrated commitment to own professional development and ongoing education.
  • Demonstrated ability to broker and coordinate community resources.
  • Ability to set priorities, organize and coordinate activities.
  • Ability to facilitate / coordinate referrals and applications to government and / or community agencies.
  • Demonstrated skills in program development, implementation and evaluation.
  • Experience in providing services to clients with chronic mental illness, crises intervention and addiction issues is an asset.
  • Excellent organization, communication and interpersonal skills and the ability to work effectively as a team member in a dynamic, interdisciplinary environment.
  • Demonstrated skills in providing education to health care teams and to students.
  • Demonstrated ability to measure and evaluate practice change outcomes.
  • Supports research and innovation in practice.
  • Ability to support and assist health care teams facing change and stress in environment.
  • Demonstrates understanding of, compliance with, and commitment to, patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Ability to perform the essential duties of the job.
