We are searching for aPermanent Full Time Social Worker to join our new West Bedford Transitional Healthteam based in Bedford Nova Scotia.

The Social Worker is integral in supporting the goal of returning to home / community by being engaged in interprofessional collaboration offering evidence based clinical expertise to inform care plans that fit with patient / family goals using effective communication strategies shared decision-making and mutual support.

The Social Workerfocuses on relationships between people and their environments which are integral to psychosocial emotional and spiritual Workers build upon the strengths of patients / families by focusing on existing coping strategies and problem-solving skills past successes and personal resources.

The Social Worker supports a commitment to ongoing quality improvement continuous exploration and implementation of new approaches learning teaching and mentoring.

About you

Previous experience supporting admission and discharge of vulnerable populations with complex social situations; advising on care plans and implementing resources giving the patient / family every opportunity to return to home / community.
