Alliance for Healthier CommunitiesToronto, ON, CanadaThe South East Grey Community Health Centre is a primary care facility situated in the beautiful community of Grey Highlands. SEGCHC is dedicated to meeting the broader needs of the communities in which we serve through the development and implementation of primary care, health promotion, and community development best practices.
Effective immediately, we are seeking a permanent full-time (37.5 hours per week) Primary Care Nurse Practitioner to work from our Markdale and Chatsworth locations. This individual must be a forward thinker who can think outside of the box to provide problem-solving strategies and contribute to the highest performing Community Health Centre in the South West region.
Reporting to the Director of Health Services and Executive Director and working as part of a Community Health Centre multi-disciplinary team, the NP’s responsibilities will include :
- Working collaboratively with a multi-disciplinary team including physicians, other nurse practitioners, nurses, admin staff, dietitians, physiotherapists, social workers, chiropodist, health promoter, and community developer.
- Working with the allied health team to develop community-based programs.
- Rostering patients and supported by a consulting physician and advanced NPs.
- Documenting all encounters on an electronic medical records system ensuring that data quality standards for charting are met.
- Must have current registration with and be a member in good standing with the College of Nurses of Ontario, with a minimum of 3-5 years’ NP work experience.
- Primary health care experience, preferably in a community setting. Program development experience considered an asset.
- Experience serving the Mennonite and / or Amish communities would be an asset.
- Advanced proficiency in the use of computers and various software applications including Microsoft Office and electronic medical records systems (Practice Solutions would be considered an asset).
- Demonstrated excellent problem-solving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interdisciplinary team environment.
- Experience in and / or a strong understanding of the CHC model of care.
- Experience and willingness to provide care for the geriatric population, palliative care, and home care.
